摘 要:目的通过对现有内涵的评价,澄清对临床能力内涵的认识。方法通过文献检索、研究对象筛选、代表性概念选择、独立内涵的提取、内容分析评价等环节对现有的临床能力内涵进行梳理。结果共提取出7条相对独立的临床能力内涵和4条护理临床能力或护士临床能力内涵,其中4条表述从广义或狭义层面上较合理地揭示了临床能力的本质;现有内涵表述中存在的主要局限和缺点有:对能力本质的表述不当;不能正确解释临床的本质;对临床能力的形成途径或构成要素解释不当;对临床能力的主体界定不恰当或过于局限;表述存在语法或逻辑错误;同语反复,用包含"临床"的词项来解释临床能力。结论建议临床能力内涵的广义表述为:临床能力是指医护人员在医疗及护理服务机构中为服务对象提供医疗护理服务的能力;狭义表述为:临床能力指医护人员顺利完成以患者为中心的各项诊疗护理活动所必需的能力。Objective To clarify people's cognition about the connotation of clinical competence through appraising the existing evi- dence. Methods To clear up the existing evidence concerning the connotation of clinical competence, we retrieved related literature, screened out the research objects, selected the representative concepts, extracted independent connotation, and appraised the con- tents, etc. Results Seven independent connotations of clinical competence, four connotations of nursing clinical competence or nurse's clinical competence were extracted, four of which gave generalized or narrow sense to clinical competence rationally. Limits of the existing interpretion include: inappropriate description of the essence of competence; failure to correctly interpret the essence of clinical; inappropriate interpretion of ways and factors of clinical competence;limited or inappropriate definition of the principal part of clinical competence;syntax or logical errors in descriptions;tautology: using words containing clinical to interpret the conno tation of clinical competence. Conclusion It is suggested that the generalized connotation of clinical competence is the capability that medical staffs possess to provide medical or nut-sing service to the objects in medical or nursing facilities, and the narrow sense of it is the necessary capability that medical staffs possess to be engaged in such activities as diagnosis, treatment and nursing care.
