无囊膜眼后房型人工晶体巩膜层间无缝线固定术临床观察

摘　　要：目的：探讨在无囊膜的特殊情况下，二期后房型人工晶状体（IOL）植入手术方法。方法：对19例（19眼）患者无后囊膜支持的情况下进行二期后房人工晶体植入。采用巩膜层间无缝线人工晶体固定术，三片式人工晶体折叠后由透明角膜切口注入，晶体襻由角膜缘后2mm的巩膜瓣下穿刺口穿出，置人预先做好的巩膜层间穿刺隧道内。术后随访半年以上，观察眼压、视力、人工晶体稳定性及并发症。结果：术后19例患者视力均明显提高，视力〉0．6者13例，0．2～0．6者6例，眼压稳定，人工晶体居中良好，无偏斜及移位，随访期间无晶体脱位，眼内炎、低眼压、角膜失代偿等严重并发症。结论：在晶状体囊膜缺失眼，巩膜层间无缝线固定人工晶体术，是一种可行有效的手术。Objective： To investigate the surgery of secondary posterior chamber intraocular lens implantation in special cases of capsule coloboma.Method： Nineteen eyes of 19 patients with capsule coloboma underwent secondary posterior chamber intraoeular lens implantation which were fixed in the intra sclera and sutureless, the folded three-pieces of intraocular lens were injected into the posterior chamber through the incision on the clear cornea, the loops passed out though the scleral flap 2 mm after the limbus and went into the pre-made seleral tunnel incision.Postoperative followupineludedintraocularpressure, best-correctedvisuaIacuity, IOL stability, and other postoperative complications for over 6 months.Resuit： Nineteen eyes of 19 patients were involved in the study, visual acuity after surgery was better than 0.6 in 13 eyes, between 0.2 and 0.6 in 6 eyes, all patients experienced obvious vision improvement with normal intraocular pressure, the IOLs were centered without tilt or decentration, no severe complications such as endophthalmitis, low intraocular pressure, coneal decompensatory were observed during the follow-up.Conclusion： Sutureless intraseleral posterior chamber intraocular lens implantation is a reliable and effective therapy for eyes without posterior capsular support.
