经颅多普勒对老年外伤性脑血管痉挛的诊断价值

摘  要:目的 评价经颅多普勒对老年外伤性脑血管痉挛的诊断价值.方法 选取老年外伤性脑血管痉挛患者47例为研究组,同时抽取同期非脑血管痉挛的脑损伤患者48例为对照组.研究组进行脑动脉血流速度检测,采用多普勒(TCD)检测不同GCS得分患者的VMCA、VICA、RI等指标.同时测定2组患者的PI(脉搏指数)并作比较.结果 研究组患者的动脉血流速度在2周内出现由上升到高峰再下降的变化,5~7 d血流速度达到高峰,与其他时间段相比差异明显;研究组不同GCS得分患者VMCA、VICA、RI比较差异明显;2组PI差异明显(P均<0.05).结论 经颅多普勒对老年外伤性脑血管痉挛的诊断有一定意义.Objective To evaluate the application value of transcranial Doppler in diagnosis of the elderly traumatic cere- bral vasospasm. Methods 47 elderly patients with traumatic cerebral vasospasm were selected and included in the research group, at the same time, 48 patients with cerebral injury but not the cerebral vasospasm in the same period were selected ran- domly and included in the control group. 47 patients in the research group were tested the blood flow velocity of the cerebral ar- tery; TCD was used to test the VMCA and VICA, RI and other indexes of the patients with different GCS. Simultaneously, the PI (pulse index) of 47 patients in the research group and 48 patients in the control group were tested and compared. Results The blood flow velocity of cerebral artery for patients in the research group first went up to peak and then declined in two weeks, and the peak came within 5-7 d, which was significantly different with the rest of the period; the VMCA and VICA, RI indexes of the patients with different GCS existed significant difference; the PI in the two groups was different obviously(P〈0.05), and had a statistical significance. Conclusion Trancranial Doppler in the diagnosis of the elderly traumatic cerebral vasospasm has certain significance.
