摘 要:目的探讨扩张皮瓣转移术后皮瓣微循环与内皮素-1(ET-1)的关系。方法用7只小型猪,模拟成扩张皮肤转移术后淤血皮瓣42个,分成7组,即术前、术后1,3,6,9,12和24h,用激光多谱勒血流测定仪测距蒂部8cm处的背部皮肤微循环血量和放免法测定远端(距蒂部8cm),近端(距蒂部1cm)皮瓣ET-1的含量,并在24h内连续观察皮瓣颜色。结果扩张皮瓣近端ET-1含量1h与术前相比有增高趋势,3h增高显著(t=2.32,P〈0.05),6h达到峰值(t=3.18,P〈0.01),随后开始下降。扩张皮瓣远端ET-1含量则无明显变化。远端淤血部微循环与近端ET-1有显著负相关(R=-0.755,P〈0.01)。结论扩张皮瓣转移术后皮瓣近端(蒂部)ET-1含量对远端微循环起重要调节作用。Objeetive To evaluate the relationship of endothelin(ET) and flap microcirculation. Methods Seven mini pigs were employed to establish expansion models and 42 expanded flap made were divided into 7 groups(before surgery, 1,3,6, 9,12,24 h after surgery). After different hours,distal microcirculation were detected by dopler and both distal and base ET level were measured by radio-immunity. Expanded flap colour were observed. Results Compared to ET-1 level before sur-gery, ET-1 levle of 1 h after surgery group increased, 3 h after surgery group had significant difference (t=2.32,. 05), 3 h after surgery group got a peak (t= 3.18,0.01), and sbusequently decreased. Distal microcirculation had statistically positive correlation with base skin ET level (R=-0. 755 ,P〈0.01). Conclusion Base ET had important effect on distal blood flow after expansion.
