摘  要:目的探讨应用Pentacam眼前节测量分析系统和超声生物显微镜(UBM)定量分析可疑房角关闭(PACS)患者激光周边虹膜切开术(LPI)术前、术后前房深度(ACD)、周边房角(ACA)、前房容积(ACV)的动态变化。方法前瞻性非随机对照临床研究。临床选取行LPI术的33例PACS患者,术前、术后1周分别用Pentacam和UBM测量眼前节数据。比较两种方法测量结果的差异。结果①Pentacam测量结果,术后ACD无显著性变化(t=-0.170,P>0.05),ACA加宽(t=-2.218,P<0.05),ACV加大(t=-4.879,P<0.05)。②UBM测量结果,术后ACD无显著性变化(t=0.1,P>0.05),ACA加宽(t=-8.258,P<0.05)。③Pentacam与UBM在测量术前、术后ACD、ACA值时,差异有统计学意义(t=-7.050,t=-15.038,t=10.451,t=2.274,P均<0.05)。在测量ACD时呈正相关,测量ACA时不具有相关性。应用Bland-Altman分析,两种方法测量ACD、ACA具有较好的一致性。结论 Pentacam作为一种新型、无创的三维眼前节分析系统,可以定量分析PACS患者LPI术后前房结构的变化,与UBM比各有优势,其值有一定差异,ACD值和ACA值有良好的一致性。在临床工作中可发挥两种仪器各自优势使用。Objective To explore the application of Pentacam anterior segment measurement analysis system and ultrasonic bio microscope (UBM) quantitative analysis in the primary angle closure suspect (PACS) patients preoperative and postoperative of laser peripheral iridotomy (LPI) anterior chamber depth (ACD) , ambits chamber angle (ACA) and anterior chamber volume (ACV) dynamical change. Methods This is a prospective non randomized controlled clinical study. 33 patients from PACS were included into this study. All these patients received LPI operation using Pentacam and UBM to measure the patients'anterior segment data of preoperative and postoperative one week. The measuring re suits were compared in the difference between the two methods. Results (1)Result of Pentacam measurement shows there were no significant change of ACD ( t O. 170, P 〉 0.05 ), ACA significant change widen ( t = 2.218, P 〈 0. 05 ), ACV significant change larger ( t = 4. 879, P 〈 0.05 ) at postoperative. )Result of UBM measurement shows that postoperative were no significant change of ACD ( t = 0. 170, P 〉 0.05 ), ACA significant change widen ( t = 2. 218, P 〈 0. 05 ). (3) Analysis the ACD and ACA preoperative and postoperative data which were measured by Pentacam and UBM shows that the difference of ACD and ACA have statistic changes ( t = 7. 050, t = 15. 038, t = 10. 451, t = 2. 274 and all P 〈 O. 05 ), and ACD were positively correlated, ACA is not relevant. Bland Altman analysis shows that two methods of measur ing ACD, ACA have good consistency. Conclusion Pentacam, as a new, noninvasive three dimension eye anterior segment analysis system, can be used in quantitative analysis anterior chamber structure changes of PACS patients LPI preoperative and postoperative. And there are certain differences compared with UBM. And the measure results of ACD and ACA have good consistency, which can play their respective advantages of both instruments to use in the clinical work.
