部分边缘外肿瘤切除结合病灶内刮除治疗股骨远端骨巨细胞瘤的疗效分析

摘　　要：目的探讨股骨远端骨巨细胞瘤部分边缘外肿瘤切除+关节面下刮除+自体骨植骨钢板内固定术治疗股骨远端骨巨细胞瘤的临床疗效。方法选择2010年6月—2012年6月收治的13例股骨远端骨巨细胞瘤患者,均行部分边缘外肿瘤切除+关节面下刮除+自体骨植骨钢板内固定术。采用膝关节Bristol评分评估术后膝关节功能,并随访影像学资料评估是否复发。结果 13例患者均顺利完成手术,且术后均获得随访,随访时间0.5～2.5a,中位随访2a。随访期间均未发现复发,Bristol膝关节评分:术后6个月为(39.1±5.5)分,术后1a为(42.4±2.8)分,术后2a为(43.6±3.3)分。结论部分边缘外肿瘤切除+关节面下刮除+自体骨植骨钢板内固定术是治疗股骨远端良性肿瘤的一种有效的手术方法。Objective To investigate the clinical effectiveness of the treatment of partly wide resection combined with curettage,autogenous bone graft and internal fixation on gaint cell tumor located in distal femo-ral.Methods Retrospectively collected thirteen cases of gaint cell tumors located in distal femoralfrom June 2 0 1 0 to June 2 0 1 2 .Partly wide resection combined with curettage,autogenous bone graft and internal fixation was applied for all the patients.Postoperation knee joint function was evaluated according to the Bristol scoring system.Imaging data was used to assess recurrence during follow-up.Results The effect of surgery was good. All patients received 0 .5-2 .5 years follow-up and median follow-up was 2 years.Recurrence was not foundin all patients during the followed-up.Bristol scoring showed that the score for 6 months after surgery was （3 9 .1 ± 5 .5 ）and score for 1 year after surgery was （4 2 .4 ± 2 .8 ）and score for 2 years after surgery was （4 3 .6 ± 3 .3 ）. Conclusion Partly wide resection combined with curettage,autogenous bone graft and internal fixation is an effective surgical method to treat gaint cell tumor in distal femoral.
