无缝线骨髓间充质干细胞羊膜移植预防角膜缘干细胞缺乏的实验研究

摘　　要：目的探讨无缝线骨髓间充质干细胞(bone marrow mesenchymal stem cells,BMMSC)羊膜移植在兔角膜缘干细胞缺损模型的应用效果。方法选择新西兰白兔36只(36眼,均为右眼),制作兔角膜缘干细胞缺损模型,模型制作后第2天随机分为三组:实验组(A组)行无缝线BMMSC羊膜移植术,阳性对照组(B组)行单纯羊膜移植术,阴性对照组(C组)不做手术处理。分别于术前和术后1 d、2 d、3 d、7 d、14 d、28 d观察眼压、角膜新生血管、角膜透明度、前房反应、泪液分泌及房水中血管内皮生长因子(vascular endothelial growth factor,VEGF)浓度情况;术后7 d和28 d每组各处死3只兔子,摘除右眼作病理切片检查,HE染色观察比较炎症细胞和角膜上皮细胞形态并进行细胞计数。结果 A、B两组兔术前眼压、角膜新生血管、角膜透明度、前房反应及泪液分泌情况比较差异均无统计学意义(均为P>0.05),术后眼压、前房反应和并发症在组间各时间点比较差异均无统计学意义(均为P>0.05)。A、B两组兔术后28 d,泪液分泌、角膜新生血管和角膜透明度组间比较差异均有统计学意义(t CNV=6.746,t CT=9.262,t SIT=9.157,均为P<0.05),角膜上皮数差异有统计学意义(t=7.726,P<0.05),炎症细胞数差异也有统计学意义(t=8.126,P<0.05)。A、B两组术后各时间点房水中VEGF浓度差异均无统计学意义(均为P>0.05),与C组比较差异均有统计学意义(均为P<0.05)。结论无缝线BMMSC羊膜移植能有效预防兔角膜缘干细胞缺乏,且炎症反应小,角膜透明度高。Objective To evaluate the effects of sutureless bone marrow mesenchymal stein cells（BMMSC） with amniotic Ineinbrane transplantation for corneal limbus stein cell defect in rabbit model. Methods Totally 35 rabbits （35 eyes） with corneal liinbus stein cell defect defect model were studied, which were divided into three groups at postoperative 2 days：the experimental group （group A, the sutureless BMMSC with amniotic membrane transplantation surgery） ,positive control group（group B,the tradi- tion amniotic membrane transplantation surgery） and negative control group（ group C, without surgery）. The postoperative examinations including the intraocular pressure （ IOP）, corneal new vessels （ CNV）, corneal transparence （ CT ）, reaction of the anterior chamber,tear film function, complications and VEGF concentration in aqueous humor were performed at pre-operation and postoperative 1 day,2 days ,3 days,7 days, 14 days and 28 days, respectively. Three rabbits in each group were killed at postoperative 7 days and 28 days, the right eyeballs were extirpated to observe the condition of corneal epithelium cells and inflammatory cells by HE staining, then coinpared their difference. Results There were no statistical difference in the symptom of preoperation IOP, CNV, CT, Schirmer I test（ SIT）, reaction of the anterior chamber and complications be- tween group A and B （ all P 〉 0.05 ）, no statistical difference in postoperation IOP and reaction of the anterior chamber and complications （ all P 〉 0.05 ）. After 28 days, there were statistical difference in CNV, CT, SIT（ tcNv = 6.745, tcT= 9.252, tsrr = 9. 157 ; all P 〈 0.05 ）, and statistical difference in corneal epithelial cell number and inflammatory cell number between group A and B（ t =7. 725,8.126,both P〈0.05）.
