光学相干断层扫描在测量8~12岁国人正常眼视网膜神经纤维层厚度中的应用

摘　　要：目的探讨视盘周围视网膜神经纤维层（retinal nerve fiberlayer，RNFL）厚度各相关参数与性别、年龄、眼别、民族的关系。方法应用光学相干断层扫描对8～12岁正常国人214例359眼正常眼进行研究，采用RNFL厚度分析程序，测量以视盘为中心直径3．4mm的全周以及不同象限和不同钟点位的RNFL厚度。所得资料应用SPSS19．0软件进行统计学处理，分析性别、年龄、眼别、民族等多个因素对RNFL厚度的影响。结果8、9、10、11、12岁各年龄段受检者的全周RNFL厚度分别为（108．32±9．42）μm、（108．52±8．78）μm、（112．36±11．14）μm、（110．27±10．21）μm、（113．11±8．81）μm，8～12岁男性受检者的全周RNFL厚度为（110．15±9．34）μm，女性为（109．97±10．01）trm；汉族受检者的全周RNFL厚度为（111．34±9．81）μm，非汉族受检者的全周RNFL厚度为（109．78±11．03）μm。右眼全周RNFL厚度为（110．41±9．89）μm，左眼全周RNFL厚度为（109．63±9．92）μm。8～12岁各年龄组间比较，以及男、女比较，左、右眼比较、汉族与非汉族比较全周RNFL厚度差异无统计学意义（均为P〉0．05）。结论本研究发现8—12岁正常我国儿童性别、年龄、眼别、民族对该年龄段RNFL厚度无明显影响。检查结果与成人测量值有明显差异，涉及RNFL改变的疾病诊断和随诊时不应使用成人标准。Objeetive To investigate the relationship of related indexes of retinal nerve fiber layer（RNFL） thickness around the optic disc with gender, age, eye side, and ethnic group. Methods A total of 359 eyes from 214 normal Chinese aged from 8 years old to 12 years old were recruited for this study. The RNFL thickness at different part-time bit, quadrantand and global average RNFL thickness around the optic disc were measured by optical coherence tomography（OCT） with 3.4 nun diameter circle, using the RNFL thickness average analysis program. The data was analyzed with SPSS statisticas 19.0 software. The effects of several factors（ such as gender,age, eye side and ethnic group） on RNFL thickness were also analyzed, Results The global average RNFL thickness at 8 years old,9 years old,10 years old,ll years old and 12 years old were（ 108.32 ± 9.42 ）μm, （ 108.52 ± 8.78 ） pan, （ 112. 36 ±11. 14 ） μm, （ 110. 27 ± 10. 21 ）μm, （ 113. 11±8.81 ）μm, respectively. All of the male＇ s average RNFL thickness was（ 110.15 ±9.34） μm, and female＇ s average RNFL thickness was（ 109.97 ± 10.01 ）μm. The average RNFL thickness of Han nationality was（ 111.34±9.81 ）μm, and non-Han nationality was（ 109.78 ± 11. 03 ） μm. RNFL thickness of right eyes was（ 110.41 ± 9. 89）μm,and left eyes was（ 109.63 ± 9.92） μm. There was no significant difference in RNFL thickness between different gender, age, eye side and ethnic group （ all P 〉 0.05 ）. Conclusion Gender, age, eye side and ethnic group has no effect on their RNFL thickness in children aged from 8 years old to 12 years old,which has significant difference compared with adult＇ s data. It shouldn＇ t use the adult＇ s data for diagnosis and follow-up of diseases involving in RNFL changes.
