玻璃体注射雷珠单抗治疗缺血型视网膜中央静脉阻塞的临床观察

摘　　要：目的观察玻璃体注射雷珠单抗治疗缺血型视网膜中央静脉阻塞的临床疗效及安全性。方法选择2012年7月至2013年3月就诊我院眼科门诊且明确诊断为缺血型视网膜中央静脉阻塞患者36例36眼，均给予玻璃体注射雷珠单抗注射液，并在治疗前及治疗后1～2个月内行视力、裂隙灯、眼底镜、眼底荧光血管造影（fundus fluoresecine angiography，FFA）、光学相干断层扫描（optical coherence tomography，OCT）检查，对比分析治疗前后患者上述检查指标变化情况。结果36例36眼患者中，术后视力提高〉3行者27例27眼，提高1—3行者6例6眼，视力无提高者3例3眼，总有效率为91．7％。与治疗前比较，FFA显示视网膜渗漏明显减轻．OCT示视网膜下、视网膜内或视网膜色素上皮下积液明显吸收。治疗后所有患者黄斑中心视网膜厚度为176～427（263．69±87．28）μm，与治疗前相比减轻（t=9．763，P〈0．05）。随访过程中未见与药物有关的眼部和全身不良反应。结论玻璃体注射雷珠单抗治疗缺血型视网膜中央静脉阻塞，可快速、稳定地提高患者的视功能，其安全性高、耐受性好，并能及时有效预防并发症。Objective To observe the clinical effects and safety of intravitreal injection of lucentis for ischemic central retinal vein occlusion. Methodu Thirty-six eyes of 36 eases diagnosised as chemic central vein occlusion definitely were intravitreally injected lucentis from July 2012 to March 2013 in our hospital, the visual acuity examination,slit lamp examination, funduscopes examination, fluorescein fundus angiography and optical coherence tomography were performed before and one month to two months after treatment, Results In the 36 eyes, the visual acuity of 27 eyes improved 3 or above 3 lines,5 eyes improved 1 line to 3 lines,3 eyes were ineffective,the total efficiency was 91.7%. Compared with preoperation, fluorescein fundus angiography indicated leakage of retina was aUeviative obviously, optical coherence tomography indicated the hydrops of ineretina,subretina,subretinal pigment epithelium was assimilated obviously after operation. The postoperative thickness of macular central fovea was 175 - 427 （263.69 ± 87.28 ） μm,there was statistical difference compared with preoperation（ t = 9.753 ,P 〈 0.05 ）. In the follow-up ,no adverse reaction of ocular and body related to drug was found. Conclusion Intravitreal injection of lucentis for ischemic central retinal vein occlusion can improve visual function rapidly and steadily with high safety and good tolerance, and can prevent complications effectively and timely.
