共同性斜视矫正术前后眼压变化的临床观察

摘　　要：目的观察共同性斜视矫正术前后眼压变化。方法 34例(65眼)接受共同性斜视矫正术患者,按手术涉及的眼外肌分组:一条水平直肌后退组共44眼,其中单纯一条内直肌后退组19眼,单纯一条外直肌后退组25眼;一条水平直肌后退联合同眼另一条水平直肌缩短组21眼。检查术眼的术前和术后1 d、2 d、3 d、7 d、2周和1个月的眼压,并进行统计学分析。结果单纯一条内直肌后退组患者术前眼压为(16.53±3.56)mmHg(1 kPa=7.5 mmHg),术后1 d、2 d、3 d、7 d、2周、1个月分别为(19.52±3.76)mmHg、(19.22±2.66)mmHg、(19.45±2.81)mmHg、(16.36±3.60)mmHg、(15.61±2.16)mmHg、(15.50±3.11)mmHg;术后1 d、2 d、3 d眼压均较术前升高,差异均有统计学意义(均为P<0.05);术后7 d、2周和1个月眼压均接近术前水平,差异均无统计学意义(均为P>0.05)。单纯一条外直肌后退组患者术后1 d、2 d、3 d眼压均较术前升高,差异均有统计学意义(均为P<0.05);术后7 d、2周和1个月眼压均接近术前水平,差异均无统计学意义(均为P>0.05)。一条水平直肌后退联合同眼另一条水平直肌缩短组患者术后1 d、2 d眼压均较术前升高,差异均有统计学意义(均为P<0.05),术后3 d和7 d眼压与术前差异均无统计学意义(均为P>0.05);术后2周和1个月眼压下降,与术前比较差异均有统计学意义(均为P<0.01)。结论共同性斜视手术中单纯剪断一条水平直肌,仅导致眼压早期升高,术后7 d基本恢复至术前水平;剪断两条水平直肌,导致术后早期眼压升高,之后逐渐下降。Objective To investigate the changes of intraocular pressure （IOP） before and after commitant strabismus operation. Methods Thirty-four commitant strabismus cases（55 eyes） were grouped in accordance with methods of operation： a horizontal rectus recession group （44 eyes ）, including internal rectus muscle recession group （19 eyes）, external rectus muscle recession group （25 eyes）, an external rectus recession and a horizontal rectus resection group（21 eyes）. IOP of these eyes were meas- ured pre-operation and 1 day, 2 days, 3 days, 7 days, 2 weeks and 1 month after oper- tion. Then the data were analyzed. Results In the internal rectus muscle recession group,preoperative IOP and postoperative IOP at 1 day, 2 days, 3 days,7 days, 2 weeks and 1 month were （ 15.53 ± 3.56 ） mmHg （ 1 kPa = 7.5 mmHg）, （ 19.52 ± 3.76 ） mmHg, （19.22 ±2.66）mmHg, （ 19.45±2.81 ） mmHg, （ 16.36 ±3.60） mmHg, （ 15.61 ±2.16） mm Hg, （ 15.50 ± 3. 11 ） mmHg, respectively, there were statistical differences between preoperation and 1 day,2 days, 3 days after operation （ all P 〈 0.05 ）, no statistical difference between pre-operation and 7 days, 2 weeks, 1 month after operation （ all P 〉 0.05 ）. In the external rectus muscle recession group,there were statistical differences in IOP between pre-operation and 1 day,2 days ,3 days after operation（ all P 〈 0.05 ） ,no statistical difference between pre-operation and 7 days, 2 weeks, 1 month after operation （ all P 〉 0.05 ）. In the external rectus muscle recession group and the horizontal rectus recesday after operation （ all P 〈0. 05 ）, no statistical difference between pre-operation and 3 day, 7 days after operation （ all P 〉 0. 05 ）, and statistical difference between pre-operation and 2 weeks, 1 month after operation （ all P 〈 0. 01 ）.
