摘  要:目的 探讨肝动脉化疗栓塞 (TACE)联合射频消融(RFA)对兔肝VX2残余肿瘤血管生成的影响。方法 CT引导下经皮穿刺建立45只新西兰大白兔VX2肝癌模型,3周后荷瘤兔肝肿瘤直径为2.5 ~ 2.7 cm,随机分成对照组(假性治疗组)、单纯RFA组和TACE联合RFA组(联合组)。于治疗后1、4、7 d时每组各处死5只瘤兔,取肿瘤中心组织及边缘组织,用免疫组化法检测肿瘤组织血管内皮生长因子(VEGF)和微血管密度(MVD)表达。结果 单纯RFA及联合治疗组消融区周边的肿瘤组织VEGF和MVD表达均明显低于对照组(P 〈 0.05)。联合治疗组VEGF和MVD绝对值略高于单纯RFA组,但差异无统计学意义(P > 0.05)。残余肿瘤细胞VEGF与MVD表达呈正相关(P 〈 0.01)。结论 单纯RFA和RFA联合TAE治疗兔肝VX2肿瘤后1周内均能抑制肿瘤血管生成,且联合治疗在抑制肿瘤血管生成作用方面与单纯RFA治疗作用相当。Objective To discuss the influence of transcatheter arterial chemoembolization (TACE) together with radiofrequency ablation (RFA) on the angiogenesis of remnant tumor in rabbit models with hepatic cancer. Methods VX2 hepatic tumor model was established in 45 New Zealand white rabbits through CT-guided percutaneous implantation of VX2 tumor cells into the liver. Three weeks later the tumor size was (2.5 - 2.7) cm in diameter. The experimental rabbits were equally and randomly divided into control group (pseudo- therapy group), simple RFA group and combination group (TACE with subsequent RFA). Every 5 rabbits from each group were sacrificed each time at one, 4 and 7 days after the treatment. Both the central and peripheral tissue of the ablated tumor were collected and sent for pathological examination with immunohistochemical method to determine the expression level of vascular endothelial growth factor (VEGF) and micro-vessel density (MVD). The results were analyzed by using SPSS13.0 software. Quantitative data were showed by the equation of (x -+ s), and P 〈 0.05 was defined as the threshold for statistical significance. Results The expressions of VEGF and MVD of tumor's peripheral tissue in both RFA group and combination group were significantly lower than those in control group (P 〈 0.05 ). The absolute values of VEGF and MVD in combination group were slightly higher than that in RFA group although the difference was not significant (P 〉 0.05). In the residual tumor cells VEGF expression had a parallel relationship with RFA expression (P 〈 0.01 ). Conclusion Both simple RFA therapy and RFA combined with TACE therapy can effectively inhibit tumor angiogenesis within one week after the treatment. Besides, with respect to the inhibition of tumor angiogenesis combination therapy has quite the same effect as simple RFA therapy does.
