单操作孔胸腔镜下微创治疗纵隔肿瘤22例体会

摘　　要：目的:分析单操作孔胸腔镜下微创治疗纵隔肿瘤的临床效果。方法:对22例行胸腔镜手术的纵隔肿瘤患者相关资料进行回顾分析。结果:22例均完全在胸腔镜下完成手术,16例在单孔操作下完成,6例因游离困难加开副操作孔,手术时间30-110min,平均65min;术中出血30-200ml,平均100ml;术后胸管引流时间2-4天,平均3天;术后住院时间5-8天,平均6.5天。均顺利恢复出院,无术后出血、气胸、感染、肺不张等并发症。随访1-20个月,未有局部复发。结论:单操作孔胸腔镜下切除纵隔肿瘤可达到双操作孔的手术效果,进一步减少手术创伤,加快术后恢复,可在临床上加以推广应用。Objective：To analyze the clinical effect of uniportal video-assisted thoracoscopic surgery （VATS） for minimally invasive in the treatment of mediastinal tumor.Methods：Retrospectively analyzing the clinical information of 22 cases of mediastinal tumor treated by video-assisted thoracoscopic surgery.Results：All surgeries were carried out successfully.Among the 22 cases,16 received uniportal VATS and the other 6 received two-portal VATS.The average operation time was 65min,average blood loss was 100ml,average drainage time was 3 days,average postoperative hospital stay was 6.5 days.All the patients achieved postoperative recovery and no severe complication occurred.The follow-up time ranged from 1 to 20 months and there was no recurrence.Conclusion：Uniportal VATS has the same effect as two-portal VATS in the treatment of mediastinal tumor.The surgical trauma was less than two-portal VATS,and can accelerate patients＇ postoperative recovery,should be extended in clinical treatment.
