Twist、N-cadherin表达与移行上皮肿瘤恶变及病理参数的关系

摘　　要：目的:研究上皮间质转换相关基因蛋白Twist、N-cadherin在移行细胞癌变中的作用。方法:采用免疫组化SP法检测Twist和N-cadherin在移行上皮乳头状瘤、具有恶性潜能的乳头状瘤、低级别尿路上皮癌、高级别尿路上皮癌中的表达,并与其临床病理参数进行对比研究。结果:Twist、N-cadherin在移行上皮乳头状瘤、具有恶性潜能的乳头状瘤、低级别尿路上皮癌、高级别尿路上皮癌中的阳性表达率随病变级别升高而明显增加;表达水平与病变程度间存在明显正相关(r=0.6791,P=0.0021;r=0.6612,P=0.0001)。Twist在尿路上皮癌中表达水平与组织学类型、浸润深度、淋巴结转移密切相关(P<0.05),与肿瘤体积、TNM分期无关(P>0.05);N-cadherin表达水平与肿瘤体积、组织学类型密切相关(P<0.05),与淋巴结转移、TNM分期、浸润深度无关(P>0.05)。Twist和N-cadherin在移行细胞肿瘤不同病变中表达水平的相关性分析显示,两者表达存在明显正相关(r=0.6723,P<0.05)。结论:Twist和N-cadherin在移行细胞癌变过程中明显升高,同时两者共同参与移行细胞癌局部浸润和淋巴结转移。Objective：To study the role of epithelial-mesenchymal transition associated gene protein Twist,N-cadherin in transitional cell tumor.Methods：Using immunohistochemical SP method to examine Twist and N-cadherin expression in the malignant potential of transitional cell papilloma,papilloma,low-grade urothelial carcinoma,urothelial carcinoma in high-level,and its clinicopathological parameters were analyzed.Results：The positive expression ratio of Twist N-cadherin in transitional cell papilloma,papilloma with malignant potential,low-grade urothelial carcinoma,high grade urothelial carcinoma increased significantly with the lesion level（r =0.6791,P =0.0021,r =0.6612,P =0.0001）.There was significant relationship between the level of Twist expression in urothelial carcinoma and histological type,depth of invasion,lymph node metastasis （P ＜ 0.05）,and no relationship with tumor size,TNM stage （P ＞ 0.05）,there was significant relationship between N-cadherin expression levels and tumor volume,histological type （P ＜ 0.05）,no relationship with lymph node metastasis,TNM stage,depth of invasion （P ＞ 0.05）.There was relationship between expression level of Twist and N-cadherin in transitional cell tumors （r =0.6723,P ＜ 0.05）.Conclusion：The level of Twist and N-cadherin was significantly increased,in the process of transitional cell malignant,both of them to take part in transitional cell carcinoma＇s local invasion and lymph node metastasis.
