摘 要:目的:将细胞DNA倍体定量分析方法与TBS常规细胞学方法应用于宫颈癌及癌前病变的筛查,比较两者的诊断效果,以提高诊断的准确性。方法:对门诊及体检的3165名妇女进行宫颈癌筛查。用宫颈刷取材,液基薄层制片后,分别进行DNA染色和巴氏染色,并分别进行全自动细胞扫描诊断和TBS常规细胞学诊断,对以上检查可疑的妇女在阴道镜下行宫颈组织学活检。结果:细胞DNA倍体分析系统发现异常303例(9.57%),常规细胞学检查发现异常189例(5.98%)。135例妇女进行了活检,以病理诊断结果为标准,计算出DNA定量分析方法及常规细胞学方法在筛查CINⅡ以上宫颈病变的敏感性、特异性和阳性预测值,分别为84.21%、86.96%、84.21%和46.97%、91.30%和83.78%。结论:细胞DNA倍体定量分析方法在宫颈癌及癌前病变的筛查中具有较高的敏感性。Objective:To evaluate values of combination application of DNA ploidy analysis and routine cytology examination in the liquid-based thin-layer cytology test for cervical squamous cell carcinoma.Methods:Cytology samples were collected by cervical brushing from 3,165 out-hospital patients or healthy women for cancer screening and prepared by liquid-based thin-layer technology.The ploidy status was evaluated by Fulgen reaction and DNA cytometry,and the routine examination was performed by Barth-staining and using the Bathesda system (TBS).Biopsy was performed for the suspicious cases of neoplastic lesions.Results:DNA ploidy aberrations were found in 303 (9.57%)of the cases,and atypical squamous cells or more severe abnormalities were noted via routine cytological examination in 189(5.98%)of them.Biopsy results were retrieved for 135 cases,and sensitivity rate,specificity rate and prediction accuracies of these two approaches were 84.2%,87.0%,84.2% and 47.0%,91.3%,83.8%,respectively.Conclusion:DNA cytometry is proven to be more sensitive than the routine cytological examination in the screening program for cervical squamous cell carcinoma and its precursors.
