烧伤后康复期老年患者抑郁评分与血清P物质和神经肽Y表达的关系研究

摘　　要：目的探讨烧伤后康复期老年患者抑郁评分与血清P物质(SP)和神经肽Y(NPY)的关系。方法选取唐山市工人医院烧伤整形科2008—2011年收治的102例烧伤后康复期老年患者作为观察组,选择同期体检健康的老年人70例作为对照组,应用抑郁自评量表(SDS)对两组受试者抑郁程度进行评分,应用酶联免疫吸附法(ELISA)检测两组受试者血清SP和NPY的表达,比较两组受试者的抑郁评分、血清SP及NPY表达。结果 (1)观察组血清SP表达和抑郁评分均高于对照组,而血清NPY表达低于对照组,差异有统计学意义(P<0.05)。(2)观察组不同抑郁评分患者血清SP和NPY表达比较,差异有统计学意义(P<0.05);不同烧伤面积患者的抑郁评分、血清SP和NPY表达比较,差异有统计学意义(P<0.05)。(3)线性相关性分析显示,抑郁评分与血清SP表达呈正相关(r=0.46,P=0.0158),与血清NPY表达呈负相关(r=-0.48,P=0.0210)。烧伤面积与抑郁评分和血清SP表达呈正相关(r=0.45,P=0.0293;r=0.49,P=0.0394),与血清NPY表达呈负相关(r=-0.43,P=0.0432)。结论烧伤后康复期老年患者存在不同程度的抑郁情绪,血清SP和NPY的异常表达与患者抑郁程度相关。Objective To investigate the relationship of depression scores to serum substance P （SP） and neuropep- tide Y in elderly patients in rehabilitation period after burn. Methods A total of 102 elderly patients in post - burn rehabilitation period admitted to this hospital from 2008 to 2011 were enrolled as study group, 70 healthy subjects as control group. Depression degree was scored by self - rating depression scale （SDS） in 2 groups, SP and NPY expressions detected by enzyme - linked im- munosorhent assay （ELISA）. Depression score, serum SP, NPY expressions were compared. Results （1） The expression of SP and depression scores were higher in study group than in control group （ P 〈 0. 05 ）, that of NPY lower （ P 〈 0.05 ）. （2） There was significant difference in SP, NPY expressions in patients with different degrees of depression in study group （P 〈 O. 05 ）, and in depression score, SP, NPY expression in patients with different burn areas （P 〈 O. 05 ）. （3） By linear regres- sion analysis, depression score was positively correlated with SP expression （r = 0. 46, P = 0. 0158 ）, negatively with NPY ex- pression （ r = - 0. 48, P -- 0. 0210）. Burn area was positively correlated with depression score and SP expression （ r = 0. 45, P = 0. 0283 ; r = 0. 49, P = 0. 0394）, negatively with NPY expression （ r = - 0. 43, P = 0. 0432）. Conclusion Elderly pa- tients in post - burn rehabilitation period have different degrees of depression. The abnormal expressions of serum SP and NPY are correlated with the development of depression.
