摘  要:目的 建立猪控制型心死亡供体模型,观察模型建立过程中血流动力学及动脉血气指标的变化.方法 白色杂种猪10只,麻醉成功后静脉给予阿曲库铵(1 mg/kg)及肝素(150 ~200 U/kg),同时撤除呼吸机以建立心死亡模型,监测模型建立过程中血压、心率、平均动脉压以及动脉血气(pH值、PaO2、PaCO2以及乳酸)的变化.结果 心死亡时间为(17.40±3.37) min.撤除呼吸机5 min内,猪出现短暂升高血压、心动过速,收缩压由(113.90±11.52) mmHg(1 mmHg=0.133 kPa)升至(118.50±10.40) mmHg,心率由(108.20±4.94)次/分升至(154.90±7.06)次/分,之后血压、心率迅速降低直至心跳停搏;同时机体动脉氧分压迅速降至(17.40±3.13) mmHg,乳酸水平升高,酸中毒加重.结论 通过模拟临床控制型心死亡供体“撤除呼吸机、机体肝素化、确定死亡时间”3个阶段,成功建立了稳定、可靠、重复性高的猪心死亡模型.Objective To establish a porcine model of controlled donors of cardiac death (CDCD) and investigate the changes of hemodynamics and arterial blood gas.Methods Ten domestic crossbred pigs were anesthetized and ventilated with 100% oxygen.Once all baseline data were taken,atracurium (1 mg/kg) was administered and the ventilator was discontinued while the animal was under deep anesthesia to establish the porcine CDCD model.Meanwhile,heparin (150-200 U/kg) was administered after discontinuation of the ventilator.The changes of blood pressure,heart rate,average blood pressure and arterial blood gas were monitored every 5 min until cardiac death.Results The time to death was approximately (17.40 ±3.37) min.Within 5 min of removal of the ventilator,systolic blood pressure and heart rate were quickly increased to (118.50 ± 10.40) bpm and (108.20 ±4.94) bpm respectively.Then blood pressure and heart rate were rapidly reduced until the heart cardiac arrest.Blood gases throughout the apneic time showed a rapid hypercapnia and acidosis.Conclusion A new swine CDCD model,simulating three stages of "withdrawal of ventilation,systemic anticoagulation and determination of death",has been successfully established.
