摘  要:目的探讨组合性抗体应用在肺腺癌与鳞癌鉴别诊断中的意义。方法应用组织芯片及免疫组织化学法检测107例肺鳞癌、119例肺腺癌p40(ΔNp63)、CK5/6、TTF-1、napsinA表达情况,评价各指标检测的敏感性和特异性。结果 p40、CK5/6在肺鳞癌阳性表达率分别为93.5%(100/107)及96.3%(103/107),特异性为97.5%及90.8%。联合检测p40和CK5/6,两项指标均阳性对肺鳞癌的特异性为99.2%;P40和CK5/6任一标志物阳性对肺鳞癌的敏感性为99.1%,均高于单一检测P40和CK5/6。TTF-1、napsinA在肺腺癌阳性表达率分别为87.4%(104/119)及89%(106/119),特异性为93.5%及96.3%。结论 napsinA是肺腺癌的特异性标记,TTF-1优于napsinA。联合检测p40(ΔNp63)、CK5/6、TTF-1、napsinA表达对肺腺癌及鳞癌鉴别诊断具有较高的实用价值。Objective To investigate the value of combined antibodies in distinguishing lung adenocarcinomas from squa- mous cell carcinomas. Methods The expression of p40 (ANp63), CK5/6, TTF-1, napsinA in 119 lung adenocarcinomas and 107 squamous cell carcinomas were examined by Elivision immunohistochemical methods using tissue microarray. Results p40. CK5/6 were positive in 93.5% (100/107) and 96.3% (103/107) of lung squamous cell carcinomas, and the specificity were 97.5% and 90.8%, respectively. For lung squamous cell carcinomas, the specificity (99.2%) and sensitivity (99.1%) of the combination of p40 and CK5/6 were higher than each of them. TTF-1, napsinA were positive in 87.4% (104/119) and 89%(106/119) of lung adenocarcinomas, and the specificity were 93.5% and 96.3%, respectively. Conclusion NapsinA is' the specialist marker for lung adenocareinomas . Combined detection of p40, CK5/6, TTF-1, napsinA is valuable to distinguish poorly differentiated lung adenocarcinomas from squamous cell carcinomas.
