中药通腑泻下法治疗重症急性胰腺炎肠功能衰竭的临床研究

摘　　要：目的:观察中药通腑泻下法对重症急性胰腺炎(SAP)并发肠功能衰竭的疗效。方法:将99例SAP患者按完全随机法分为治疗组(51例)和对照组(48例),治疗组在常规西药治疗的基础上给予胰瘅1号、胰瘅2号灌胃、灌肠治疗,观察两组患者肠功能恢复情况、并发症发生率、住院天数及病死率。结果:与对照组比较,治疗组首次排便时间、腹痛腹胀缓解时间、肠鸣音恢复时间及住院时间均显著缩短(均P<0.01);治疗组治疗7 d后的APACHEⅡ评分、胰腺感染发生率均显著低于对照组(均P<0.01);治疗组急性呼吸窘迫综合征和急性肾功能衰竭的发生率以及病死率均低于对照组,但差异无显著性。结论:在常规西药治疗基础上联合应用中药胰瘅1号、胰瘅2号灌胃灌肠的通腑泻下疗法,能显著促进SAP患者肠功能的恢复,值得推广。Objective： To observe the clinical effect on the treatment of inducing defecation to cleanse the bowels through Chinese Herbs for Intestinal Failure of Severe Acute Pancreatitis （SAP）. Methods： 99 cases of SAP patients were randomly divided into two groups：Treatment group （n=51）were treated with the method of y/dan-Ⅰ gavage and ydan-Ⅱ enema. Control group （n=48）were treated with conventional therapy, the complication rate, days of hospitalization, and mortality were observed. Results： The time of initial defecation, relief of abdominal pain and distension,rugitus recovery and average duration of hospitalization were much shorter than those in Control group （P〈0.01）. Seven days after treatmentthe APACHE II scores and infection rate of pancreas of Treatment group were significantly lower than those in Control group （P〈0.01）. The incidence and mortality of acute respiratory distress syndrome（ARDS） and acute renal failure of Treatment group were lower than those of Control group but not significantly （P〉0.05）.Conclusion： The therapy of inducing defecation through Chinese Herbs of ydan-Ⅰgavage and ydan- Ⅱ enema in combination with conventional western medicine, which promote the recovery of gastrointestinal motility of the SAP patients, deserves further clinical applications.
