Pax8在女性生殖道肿瘤诊断与鉴别诊断中应用价值

摘　　要：目的探讨Pax8蛋白在女性生殖道肿瘤中的诊断及鉴别诊断作用。方法收集Mǔllerian管各种类型上皮48例和肿瘤152例,采用免疫组化SP法检测Pax8蛋白表达。结果 Pax8在输卵管上皮阳性率为100.0%(10/10)、子宫内膜上皮为100.0%(10/10)、子宫颈黏液腺上皮为35.7%(5/14)、子宫颈鳞状上皮为0(0/14);在卵巢输卵管、宫体、子宫颈的浆液性腺癌、交界性浆液性肿瘤、子宫内膜样腺癌、透明细胞腺癌均为100.0%;在卵巢输卵管黏液性肿瘤为70.0%(7/10)、子宫颈黏液腺癌为41.7%(5/12);在卵巢性索-间质肿瘤、卵巢无性细胞瘤、子宫颈鳞状细胞癌、子宫内膜鳞状细胞癌、卵巢类癌、卵巢或子宫颈的小细胞神经内分泌癌、消化道转移性癌均为0;在盆腔腺癌为100.0%(3/3)。结论 Pax8是Mǔllerian管腺上皮敏感且特异性的标志物,对女性生殖道上皮性肿瘤的鉴别诊断起重要作用。Purpose To investigate the expression of Pax8 in female reproductive tract epithelia and tumors, and to evaluate the clini- cal significance of Pax8 in the dignosis and differential diagnosis of female reproductive tract tumors. Methods The expressing of Pax8 were detected in 48 Mhllerian epithelia and 152 Mhllerian tumors by immunohistochemitry method. Results The positive rates of Pax8 expressing were 100.0% （10/10） in epithelium of fallopian tube, 100.0% （10/10） in epithelium of endometrlum, 35.7% （5/14） in mutinous epithelium of cervix, 0 （0/14） in squamous epithelium of cervix, 100.0% in serous adenocarcinomas/serous borderline tumors/endometrioid adenocareinomas/clear cell adenocarcinomas of ovary-fallopian tube/uterine corpus/cervix, 70.0% （7/10） in mucinous tumours of ovary-fallopian tube, 41.7% （5/12） in mucinous adenocarcinomas of cervix, 0 in sex cord-stromal tumors or dysgerminomas of ovary/squamous cell carcinomas of uterine corpus or cervix/carcinoids of ovary/small cell neuroendocrine carcinomas of cervix or ovary/metastatic carcinomas from digestive tract, 100.0% （3/3） in adenocarcinomas of pelvic. Conclusions Pax8 pro- tein is a sensitive and specific marker for the M^llerian glandular epithelium. It may play an important role in the differential diagnosis of epithelial tumors of female reproductive tract.
