经Melodie多通道输液泵强化剂量EP方案时辰化疗治疗晚期小细胞肺癌的疗效分析

摘　　要：目的探讨经Melodie多通道输液泵强化剂量EP方案时辰化疗治疗晚期小细胞肺癌（SCLC）的疗效及安全性。方法将75例确诊为晚期SCI。C的初治患者随机分为两组，治疗前两组患者均给予外周中心静脉置管术（PICC），时辰组采用加强剂量EP方案，常规组采用标准剂量EP方案，至少使用2个周期，治疗每2个周期评定疗效，并观察毒副反应。结果时辰组总有效率RR和疾病控制率DCR（81．1％和91．9％）均略高于常规组（68．4％和84．2％），但差异无统计学意义。时辰组和常规组的无进展生存期（PFS）分别为5．5个月和4．25个月，时辰组有增加的趋势，但两组比较差异仍无统计学意义。两组血液学毒性为白细胞减少和贫血，非血液学毒性为消化道反应、脱发和疲乏，多以1～2级为主；时辰组3～4级恶心呕吐发生率明显低于常规组，差异有统计学意义。结论经Melodie多通道输液泵强化剂量EP方案时辰化疗治疗晚期SCLC具有疗效高、毒副作用低等优点，可作为晚期SCLC的推荐化疗方案之一。Objective To evaluate the efficacy and safety of enhanced EP regimen of chrono-chemotherapy by melodie pump in the treatment of the patients with advanced small cell lung cancer （SCLC）. Methods Seventy-five initially-treated patients with advanced SCLC were randomly divided into two groups. Before treatment, peripherally inserted central catheter was given to two groups. Besides, chrono-chemotherapy group was treated with enhanced EP regimen,while routine treatment received standard dose of EP regimen. Two cycles of treatment were given at least,and the efficiency and toxic side reactions were evaluated every two cycles. Results The response rate （RR） and disease control rate （DCR） （81. 1% and 91.90/00） in chrono-chemotherapy group were slightly higher than in routine group （68.4% and 84.2%） ,without significant difference. The progression free survivals （PFS） were 5.5 months in chrono-chemotherapy group, while 4. 25 months in routine group. PFS in chrono-chemotherapy group increased slightly, but there was no statistical significance. Hematological toxicities in both groups were mainly leukopenia and anemia,and non-hematological toxicities were gastrointestinal reactions, alopecia and fatigue, mainly at grades 1-2. Besides, the incidence of nausea and vomiting in chrono-chemotherapy group at grades 3-4 was evidently lower than that of routine group,with statistical significance. Conclusion In the treatment of patients with advanced SCLC, enhanced EP regimen chrono-chemotherapy by melodie pump is characterized by high efficiency and low toxic side effects,and can be used as a recommended therapy for advanced SCLC.
