摘 要:目的:评估利用数字化根尖片作为种植术前骨质测量手段的准确性和可靠性。方法:利用根尖片术前测量上下颌、前后区共257例种植牙位的植体颈部、中部、根尖部骨密度值,对比术中术者评估的Lekholm和Zarb颌骨分类及植入扭距值,分析其相关性。结果:下颌后牙骨密度93±12高于上颌前牙、上颌后牙及下颌前牙83±28、80±23,81±14(P<0.05),后三者间差异无统计学意义。四类骨平均骨密度值(63±5)显明低于其它三类(93±32,90±21,85±24)(P<0.05)。除颈部骨密度值与植入扭距中度相关外(r=0.6734,P=0.0325),骨密度与骨分类、植入扭距间无明显相关。结论:数字化根尖片量化评估种植区骨质量精度不足、敏感度差。Objective:To evaluate the accuracy and reliability ofpresurgical bone density assessment using digital periapical radiograph in implant rehabilitation.Methods:Standard periapical radiographs of 257 implant sites including the anterior and posterior of the maxillae and mandible were obtained preoperatively.The bone density of three parts of each implant site were measured and compared the Lekholm and Zarb jaw classification based on subjective evaluation of the surgeon and the insertion torque value.The correlation between them was analyzed.Results:The mean bone density value of posterior mandibular sites was 93 ± 12,higher than that of the anterior maxilla,posterior maxilla and anterior mandible which were 83± 28,80± 23 and 81± 14,there was no significant difference between the latter three (P〈0.05).The forth type of bone density value (63± 5) was significantly lower than the other three types (93± 32,90± 21,85± 24) (P〈0.05).Except that the neck bone density value was correlated with the insertion torque (r =0.6734,P=0.0325),classification of bone and implant torque has no significant correlation with bone density.Conclusion:The method of quantitative evaluation of bone density for implant site using digital periapical radiograph was lack of precision,sensitivity.
