摘 要:目的观察Modic各型腰椎间盘髓核与终板的病理组织学改变,探讨慢性下腰痛与其结构特点的关系。方法70例腰椎间盘退行性疾病患者行椎间盘手术,术中收集椎间盘髓核、椎体终板部分组织进行病理切片和HE染色,观察病理组织结构。结果Modic各型病理学改变如下:ModicⅠ型软骨下血管化的纤维组织,软骨终板出现裂隙或破裂;ModicⅡ型脂肪组织替代正常的软骨或骨组织;ModicⅢ型:硬化骨组织替代正常的软骨或骨组织;Modic混合型:同一终板可见黄骨髓和血管化;无Modic改变则病理学无上述变化。结论腰椎间盘髓核与终板的病理组织学改变与Modic各型的特点相联系。Objective To investigate the relationship between chronic low back pain and its structural characteristics through the histopathological changes in lumbar nucleus pulposus and endplates of different Modic subtypes. Method Method The intervertebral disc tissue of endplates and nucleus pulposus in 70 cases with lumbar intervertebral disc surgery was collected. The pathologic tissue was sliced and dyed with HE method to observe the structure. Result All of modic subtypes pathological changes were as follows: there was some fractures or ruptures in cartilage endplate and fibrous tissue were found vascularized under the cartilage tissue of Modic Ⅰ. Fat tissues were replaced by normal cartilage or bone tissues in Modic Ⅱ. Sclerosing bone tissued were replaced by normal cartilage or bone tissue in Modic Ⅲ. The yellow bone marrow and blood vessels were found in one endplate of Modic mixde type. There was not such pathological changes in non-Modic subtype. Conclusion The different Modic subtypes are associated with the pathological features in lumbar nucleus pulposus and endplates.
