人类乳头瘤病毒18整合状态与宫颈癌及癌前病变的关系

摘　　要：目的:观察宫颈病变患者脱落细胞中人类乳头瘤病毒18(HPV18)亚型整合状态及其与宫颈癌及癌前病变发展的关系。方法:选取81例HPV18阳性患者的宫颈脱落细胞,根据病理学结果进行分为:对照组(健康携带者,n=8)、宫颈炎组(n=18)、宫颈上皮内瘤变(CIN)组(CIN组,n=34)及宫颈癌组(n=21);CIN组按病变程度不同分级为CINⅠ级、CINⅡ级和CINⅢ级3个亚组。采用荧光定量PCR技术检测HPV18亚型E2和E6基因,计算二者拷贝数比值,据此分析比较各组HPV18亚型DNA的存在状态和整合率。结果:宫颈病变各组均出现HPV18亚型DNA整合状态,以宫颈癌组整合率最高,与其它各组比较差异有统计学意义(P<0.01);CIN组中,CINⅢ级的整合率最高(P<0.01)。结论:HPV18整合状态发生率随着宫颈病变程度加重而呈上升趋势;提示HPV18感染的整合状态可能与宫颈病变,尤其是宫颈癌的发生发展有关。Objective:To study the integration status relation with the cervical lesions and cervical cancer occurrence,development,through observation of the HPV18integration status changes.Method:81 cases of HPV18 positive samples were selected,including 8cases of physical examination(control group),18 cases of cervicitis(cervicitis group),34 cases of cervical intraepithelial neoplasia(CIN group)and 21cases of cervical cancer(cervical cancer group).Each group of patients with cervical exfoliated cells were collected and the fluorescence quantitative PCR detection technology was used to detect its E2 and E6,according to their DNA copies ratio to determination of HPV18 subtypes existing state.Results:The integration of different HPV18 lesions was statistically significant difference in the incidence(P<0.01).HPV 18 integration rate was highest in cervical cancer group.CIN patients for each grade group was statistically significant(P<0.01).Conclusion:HPV18 integration status may occur with cervical lesions,especially associated with the development of cervical cancer.Detection of HPV18integration status may contribute to the early detection of cervical related diseases.
