摘 要:目的探讨生物学标志物乳腺上皮小黏蛋白(SBEM)、乳腺珠蛋白(hMAM)和细胞角蛋白19(CK19)在乳腺癌患者外周血循环中的表达及其作为乳腺癌外周血微转移预测指标的价值。方法采用巢式PCR技术,检测47例乳腺癌患者和45例乳腺良性瘤患者外周血有核细胞中SBEM、hMAM和CK19的表达;采用半定量PCR技术10例乳腺癌及对应的癌旁组织以及10例乳腺良性肿瘤组织中SBEM、hMAM和CK19的表达。结果 SBEM和hMAM在乳腺癌组患者外周血中表达率高于乳腺良性肿瘤组患者(P<0.05),而两组外周血CK19表达率差异无统计学意义(P>0.05)。SBEM、hMAM和CK19在乳腺癌组织、癌旁组织及乳腺良性肿瘤组组织中均呈高表达,3组患者hMAM和CK19表达量没有差异(P>0.05),SBEM在癌组组织中表达量低于癌旁组织及乳腺良性肿瘤组织组(P<0.05)。结论 SBEM、hMAM、CK19在乳腺组织中呈稳定高表达,作为乳腺癌外周血微转移生物学标志物灵敏性高,但缺乏特异性,联合检测可提高特异性。Objective To explore the expressions of biological markers including small breast epithelial mu-cin(SBEM),human mammaglobin(hMAM) and cytokeratin19(CK19) by detecting circulating tumor cells in the pe-ripheral blood of patients with breast cancer ,and the value of SBEM ,hMAM,and CK19 mRNA in predicting microme-tastases in the the peripheral blood of patients with breast cancer .Methods The expressions of SBEM ,hMAM and CK19 were detected by nested PCR in the peripheral blood of 47 cases of breast cancer and 45 cases of benign breast tumor.Semi-quantitative PCR was used to detect the expressions of SBEM ,hMAM and CK19 in 10 cases of breast cancer tissues ,10 cases of adjacent tissues and 10 cases of benign breast tumor tissues .Results SBEM and hMAM expressions in the peripheral blood of breast cancer patients were higher than that in benign breast tumor patients ( P〈0 .05 ) ,there was no significant difference in the CK 19 expression between two groups ( P〉0 .05 ) .SBEM , hMAM and CK19 were of high expression in breast cancer tissues ,adjacent tissues and benign breast tumor tissues , there was no significant difference in the hMAM and CK 19 expressions among three groups ( P〉0 .05 ) .The expres-sion of SBEM in cancer tissues was lower than those in adjacent tissues and benign breast tumor tissues ( P〈0 .05 ) . Conclusion SBEM,hMAM and CK19 show high expressions in breast tissues .As the biological markers for micro-metastases in the peripheral blood of breast cancer ,SBEM,hMAM and CK19 are of high sensitivity ,but lack of speci-ficity,and the combination detection can improve the specificity .
