不同切面声速检测对乳腺肿物的诊断价值

摘　　要：目的探讨不同切面上声速(SV)检测对乳腺肿物的诊断价值。方法对手术病理证实的232例女性患者共248个乳腺肿物行声速匹配技术检查,获得病灶在横切面、纵切面及两切面中SV较大切面上的相应SV值,与病理诊断结果相对照,构建ROC曲线进行分析。结果乳腺良恶性肿物在横切面、纵切面及SV较大切面的SV的差异均具有统计学意义(P均<0.05)。在横切面、纵切面及SV较大切面上测得的SV鉴别诊断乳腺肿物时ROC曲线下面积(AUC)分别为0.77、0.80、0.83,横切面及SV较大切面之间的差异具统计学意义(P<0.05);Youden指数分别为横切面0.48,纵切面0.55,SV较大切面0.60。应用SV较大切面,以1547m/s作为SV临界点,鉴别诊断乳腺肿物良恶性的敏感度、特异度、准确率分别为73.08%(57/78)、87.06%(148/170)、82.66%(205/248)。结论横切面、纵切面及SV较大切面上测得的SV对鉴别诊断乳腺肿物的良恶性均有一定价值,其中SV较大切面的诊断价值最高。Objective To explore the diagnostic value of sound velocity （SV） in different sections for differentiating breast solid lesions. Methods Two hundred and thirty-two patients with 248 breast neoplasms were included. By zone speed index technique, SVs of the lesions in transverse section, longitudinal section and the section with larger SV were calculated. According to pathologic findings, ROC was constructed and analyzed. Results SVs in transverse section, lon- gitudinal section and the section with larger SV were statistically different between benign and malignant breast lesions （all P〈0.05）. The area under curve （AUC） of ROC in transverse section, longitudinal section and the section with larger SV was 0. 77, 0.80, 0.83, and the Youden index was 0.48, 0. 55, 0.60, respectively. There was significant difference be- tween AUC of transverse section and the section with larger SV （P〈0.05）. The cut-off point of SV in the section with lar- ger SV was determined as 1547 m/s, and the sensitivity, specificity and accuracy was 73.08% （57/78）, 87.06% （148/ 170） and 82.66 % （205/248）, respectively. Conclusion SV in transverse section, longitudinal section and the section with larger SV could be used to differentiate benign and malignant lesions in breast. SV in the section with larger SV has best diagnostic value.
