摘 要:目的观察新型腔内射频消融导管对离体猪肝(肝实质、栓子模型)的消融效果。方法应用EMcision Habib腔内射频消融导管及RITA射频发生器对新鲜离体猪肝肝实质及栓子模型进行消融,输出功率分别为5 W、10 W、15 W和20W,消融时间分别为60s、90s和120s,观察消融灶组织凝固形态及范围。结果消融肝实质时,输出功率为10W、延长消融时间(90s延长至120s),输出功率为15W、延长消融时间(60s延长至90s)以及消融时间为60s和90s、增加输出功率(15W增加至20W)获得的消融灶长径增加(P均<0.05),而宽径增加不明显(P均>0.05)。消融栓子模型时,输出功率为10W、延长消融时间(90s延长至120s),以及消融时间为60s、增加输出功率(15W增加至20W)均可增加消融灶长径(P均<0.05),而宽径增加不明显(P均>0.05)。肉眼见所有消融灶附着处血管管壁颜色均与邻近血管管壁无差异。结论采用EMcision Habib腔内射频导管消融离体猪肝可出现明确的消融范围,且对管道壁无明显损伤。Objective To explore the ablation effect of the newly developed radiofrequency ablation (RFA) catheter in porcine liver including hepatic parenchyma and embolus model. Methods HabibTM EndoHPB catheter was used for abla tion of ex-vivo porcine liver. The energy was delivered through RITA radiofrequency ablation generator. RFA was per formed with 5 W, 10 W, 15 W and 20 W for 60 s, 90 s and 120 s, respectively. The liver specimen was sectioned along the needle track. The visualized coagulated area was measured. Results In liver parenchyma ablation, the short diameter of ablation zone was stable with different ablation condition (all P〈0.05), but the long diameter of ablation zone became bigger with prolonging ablation duration (from 90 s to 120 s with 10 W, and from 60 s to 90 s with 15 W) and enhancing output power (from 15 W to 20 W with 60 s and 90 s, all P〈0.05). In embolus model, the short diameter of ablation diame- ter was still stable (all P〉0.05), the long diameter increased with prolonging ablation duration (from 90 s to 120 s with 10 W)and enhancing output power (from 15 W to 20 W with 60 s, all P^0.05). The color of vessel wall adhere to the ablated embolus was similar to that of adjacent wall. Conclusion HabibTM EndoHPB radiofrequency catheter can produce clear ablation zone, having no damage to the surrounding vessel wall during ablation of ex-vivo porcine liver.
