细胞角蛋白18及19与肿瘤标志物联合检测对恶性肿瘤的诊断价值

摘　　要：目的探讨细胞角蛋白(cytokeratin,CK)18、19与肿瘤标志物联合检测在恶性肿瘤诊断中的临床价值。方法 132例恶性肿瘤患者为恶性肿瘤组,68例良性疾病患者为良性疾病组,50例体检健康者为对照组,比较3组血清CK18、CK19及6项肿瘤标志物水平,分析单项肿瘤标志物检测及联合检测的诊断性能。结果恶性肿瘤组血清CK18、CK19及癌胚抗原、糖链抗原19-9、125、15-3水平明显高于良性疾病组和对照组(P<0.05);CK18、CK19与6项肿瘤标志物联合检测的灵敏度(75.00%)、诊断效率(82.80%)及AUC(0.933)明显高于单项肿瘤标志物、CK18和CK19联合检测及6项肿瘤标志物联合检测(P<0.05)。结论 CK18、CK19和6项肿瘤标志物联合检测对恶性肿瘤辅助诊断有较高临床应用价值。Objective To explore the value of combined detection of cytokeratin （CK） 18, CK19 and tumor markers to the diagnosis of malignant tumor. Methods A total of 132 patients with malignant tumor （malignant group）, 68 patients with benign tumor （benign group） and 50 healthy controls （control group） were detected the levels of CK18, CK19 and six tumor markers. The performance of single tumor marker and combined assays were analyzed. Results The levels of CK18, CK19, carcinoembryonic antigen （CEA）, carbohydrate antigen （CA）19-9, CA125 and CA15-3 were significantly higher in malignant group than those in benign group and control group （P（0.05）. Compared with single tumor marker detection, the combined detection of CK18 and CK19 and the detection of six tumor markers, the combined detection of CK18, CK19 and six tumor markers had the highest sensitivity （75.00%） and diagnostic efficiency （82.80%） as well as the biggest AUC （0. 933） （P〈0.05）. Conclusion The combined detection of CK18, CK19 and six tumor markers has a high clinical value to the auxiliary diagnosis of malignant tumor.
