摘  要:目的了解四川省泸州地区不同程度宫颈病变患者人乳头瘤病毒(HPV)感染状况及其基因型分布特点,为该地区宫颈癌的防治提供理论依据。方法采用基因芯片技术对宫颈石蜡组织及液基细胞技术所获宫颈脱落细胞进行基因分型检查。结果652例宫颈样本中,HPV检出阳性率为52.45%(342/652),其中高危感染占80.99%(277/342),低危占30.12%(103/342),混合占11.11%(38/342)。最常见前5种高危型依次为:HPVl6、52、39、58和33,最常见前3种低危型依次为HPVll、6和43。20—29和40~50年龄段HPV的总感染率较高。高危型感染在各年龄段没有显著差异,低危型感染主要集中在20—29岁,显著高于其余各年龄段(P〈0.01)。结论泸州地区HPV有较高的高、低危型感染率,且具有独特的区域和年龄段分布特点,为本地区性传播疾病及宫颈癌早期诊断、治疗、预后评估及疫苗使用等提供重要的理论依据。OBJECTIVE The study was conducted to assess the infection of human papillomavirus (HPV) among patients with cervical lesions of different severity levels in Luzhou, Sichuan and to determine the distribution characteristics of geno- types of the virus, so to provide a theoretical basis for the prevention and control of cervical cancer in the area. METHODS Gene chip technology was utilized to genotype paraffin cervical tissues and cervical cells obtained by ThinPrep cytology. RE- SULTS HPV-positive rate of the 652 cervical samples was 52.45% (342/652), including 80.99% (277/342) high-risk in- fections, 30.12% (103/342) low-risk infections, and 11.11% (38/342) mixed infections. The five most common high-risk subtypes of HPV were HPV 16, 52, 39, 58, and 33, while the three most common low-risk subtypes were HPV 11, 6, and 43. Overall HPV infection rate was high among the 20-29 and 40-50 age groups. No significant difference in the positive rate of high-risk infections was observed between different age groups; low-risk infections mainly occurred in patients aged between 20-29, among which positive rate was significantly higher than that among patients in all other age groups (P 〈 0.01). CONCLUSION Infections of high- and low-risk HPVs in Luzhou area are prevalent and exhibit unique regional and age distributions. This study provides important theoretical bases for the early diagnosis, treatment, evaluation of prognosis, and use of vaccines of sexually transmitted diseases and cervical cancer in the area.
