尿核基质蛋白在膀胱癌中的临床意义

摘　　要：目的：探讨尿核基质蛋白22（NMP22）测定在膀胱癌诊断中的临床应用价值。方法：应用免疫酶标记法（ELISA）检测28例初发性膀胱癌，10例复发性膀胱癌和20例对照组尿中NMP22水平。初发性膀胱癌组中有10例患者在术后5～7d再次检测其尿NMP22。结果：初发性膀胱癌组尿NMP22的中位数为27．1149IU／mL，复发性膀胱癌组为13．001IU／mL均明显高于对照组1．6574IU／mL（P〈0．001）；初发性膀胱癌患者尿NMP22随肿瘤病理分级的递增而升高。lO例初发性膀胱癌患者术后复测尿NMP22，结果较术前明显下降。结论：尿NMP22作为膀胱癌的肿瘤标记物具有较高的特异性和敏感性，并可在一定程度上反映肿瘤的恶性程度和预后。Objective： To evaluate whether urinary nu- clear matrix protein 22 （NMP22） is a tumor marker for the diagnosis of original and recurrent bladder cancer. Methods. Urinary NMP22 values were deter- mined in 28 cases with original bladder cancer, 10 cases with recurrent bladder cancer and 20 healthy volun- teers by an enzyme-linked immunosorbent assay （ELISA） ; 10 of 28 cases with original bladder cancer were determined for urinary NMP22 value at 5-7 days postoperatively again. Results： The median NMP22 value in original bladder cancer was 27. 114 9 IU/mL, and in recurrent bladder cancer was 13. 001 IU/mL, both groups were higher than that in healthy volunteer （1. 657 4 IU/mL, P 〈0. 001）. The NMP22 value in pa- tients with original bladder cancer increased with tumor pathologic grades. For 10 cases with original blad- der cancer determined again postoperatively, the median of urinary NMP22 value decreased remarkably as compared with pre-operation. Conclusion： Urinary NMP22 is a very specific, sensitive tumor marker for the diagnosis of bladder cancer and can indicate bladder tumor＇s malignancy degree and prognosis.
