低温等离子治疗口腔颌面部肿瘤及瘤样病变的疗效及手术护理配合

摘　　要：目的探讨低温等离子射频在口腔颌面部肿瘤及瘤样病变切除术中的疗效及护理配合。方法对64例口腔颌面部的肿瘤及瘤样病变患者采用低温等离子射频切除术治疗,并采取了有效的护理措施(心理护理、一般护理、患者体位、管道护理、手术配合及饮食指导等)。术后对患者VAS评分、出血指数、肿胀指数、患者满意度的情况进行分析,根据Williams疗效标准对患者术后1周的疗效进行评定。结果 64例患者手术均顺利完成,术中出血少,无手术并发症发生。对患者术后3 h、第1-3天及术后1周进行随访,VAS评分由(4.15±0.84)下降至(1.24±0.46)分,出血指数由(0.79±1.05)分下降至(0.00±0.00)分,肿胀指数由(1.32±0.18)下降至(0.45±0.03)分,患者满意度评分由(0.75±1.03)分下降至(0.05±0.03)分。术后1周随访32例,根据Williams疗效标准进行评定:优23例,良5例,中3例,差1例,优良率为87.50%。结论低温等离子治疗口腔颌面部的肿瘤及瘤样病变患者具有创伤小、出血少、疼痛轻、恢复快及疗效好等的优点。充分的术前准备、良好的术中配合、完善的术后护理,可以有效地减少口腔颌面部的肿瘤及瘤样病变患者术后并发症的发生,提高手术的成功率。Objective To investigate the curative efficacy of low temperature plasma ablation and intraoperative nursing cooperation in oral and maxillofacial tumors and tumor-like lesions.Methods A total of 64 patients with oral and maxillofacial tumor and tumor-like lesions received low temperature plasma ablation and effective nursing care（psychological nursing, general nursing, patient position, tube care, intraoperative cooperation and dietary instruction）. The postoperative VAS scores, bleeding scores, swelling scores and patients＇ satisfaction were evaluated after operation. Curative efficacy was assessed according to Williams criteria 1 week after operation. Results All patients successfully underwent operation with minor bleeding and few complications. Patients were re-examined 3 hours, 1-3 days and 1 week after surgery. VAS scores, bleeding scores, swelling scores and satisfaction scores decreased from（4.15__.0.84 to 1.24_＋0.46）,from（0.79_l.05 to 0.00_＋0.00）,from（1.32_＋0.18 to 0.45_0.03）and from （0.75 __.1.03 to 0.05_＋0.03） ,respectively. According to Williams criteria, the treatment outcome was excellent in 23 patients, good in 5, mild in 3, and poor in 1 excellent rate 87.5%. Conclusion Low temperature plasma ablation for oral and maxillofacial tumor and tumor-like lesions has advantages ofminor invasiveness, less bleeding, mild pain, quick recovery and good efficacy. Adequate preoperative preparation, good intraoperative cooperation and perfect postoperative care can remarkably decrease the complications and increase the surgical success rates in patients with oral and maxillofacial tumors and tumor-like lesions.
