青光眼与阿尔茨海默病的相关性研究

摘　　要：目的探讨原发性闭角型青光眼（Primary close—angle glaucoma，PACG）患阿尔茨海默病（Alzheimer’s disease，AD）发病率及其关联性。方法采用前瞻性随机临床对照的方法。对300例青光眼患者（A组）进行阿尔茨海默病筛查，选择年龄、性别等人口学情况相匹配的健康对照者300名（B组）作为对照，分析并比较两组AD患者检出情况，分析PACG与AD相关性。结果PACG组含AD患者38例，占12．7％，正常组含AD患者22例，占7．3％，两组比较差异有统计学意义（P〈0．05）。两组各年龄段发病率比较差异均无统计学意义（P〉0．05）。单因素及多因素分析提示青光眼病程、严重程度与MMSE评分呈负相关（P〈0．05），与年龄、AD病程、AD严重程度呈正相关（P〈0．05），与性别、AD临床类型、AD家族史无相关性（P〉0．05）。结论PACG可能与AD的发生存在一定关系，临床监测和控制青光眼病情可为AD的防治提供实验室依据。Objective To discuss the relationship between primary angle-closure glaucoma （PACG） and Alzheimer＇s disease （AD）. Methods Totally 300 cases of PACG patients （observation group, group A） and 300 cases of non-glaucoma controls （control group, group B） were investigated with 1：1 matched cases-control study. The relationship of PACG and AD in both subjects were de- tected and compared the detection rate. Results There were 38 AD patients in Group A （12.7%） and 20 AD patients in Group B （7.3%）. There was a significant difference in the detection rate of AD between two groups （P 〈0.05）, whereas there was no significant difference in the detection rate of different age degree AD between two groups （P 〈0.05）. Logistic analysis results showed PACG history and severity were positive correlated with age, AD history and AD severity （P 〈0.05）, and negative correlated with MMSE （P 〈0.05）. There were no correlated with sex, clinical type and fami- ly history （P 〉0.05）. Conclusions PACG is possibly correlated to AD. To monitor and control the condition of glaucoma can provide a clinical basis for preventing and treating AD.
