摘　　要：目的比较不同年龄组非酒精性脂肪性肝病（non—alcoholicfattvliverdisease，NAFLD）患者的临床和病理学表现，分析不同年龄组NAFLD患者的临床特点。方法回顾性分析2008年5月一2011年12月我院收治的经肝组织学检查确诊为NAFLD的233例患者的临床资料。结果233例分成3组，其中少儿组（≤17岁）41例，青年组（18～44岁）141例，中老年组（≥45岁）51例。患者WBC、PLT和ALT随着年龄增长而降低，y-球蛋白电泳随着年龄增长而升高；少儿组NAFLD活动度积分、ALP、AST和LDH高于青年组和中老年组；中老年组ALB和CHE显著低于少儿组和青年组，TC显著高于少儿组和青年组。年龄与脂肪变性程度之间存在一定线性关系。不同年龄组中，高血压、肝区隐痛以及乏力症状的总体分布存在统计学意义。结论随着年龄增加，NAFLD患者脂肪变性程度加重，高血压、肝区隐痛以及乏力等的发生率增加。Objective To investigate the clinical features of patients with non-alcoholic fatty liver disease （NFALD） at differ- ent ages by comparing the clinical and pathological manifestations of NFALD patients of different age groups. Method The clinical data of 233 NAFLD patients diagnosed by liver biopsy in our hospital from May 2008 to Dec. 2011 were analyzed retrospectively. Results The patients were divided into 3 groups, child and adolescent group （≤ 17 years, n=41）, youth group （18-44 years, n=141） and middle-aged and older adult group （≥45 years, n=51）. WBC, PLT and ALT decreased, while -EP increased with age. NAFLD activity score and the levels of ALP, AST and LDH were significantly higher in child and adolescent group than in youth group and middle-aged and older adult group. The levels of ALB and CHE were significantly lower and TG level was significantly higher in middle-aged and older adult group than in child and adolescent group and youth group. There was a linear relationship between age and hepatic steatosis. There were significant differences in the occurrence of hypertension, liver pain and fatigue among different age groups. Conclusion NAFLD patients present with severer hepatic steatosis and increased incidence of hypertension, liver pain and fatizue with axe.
