全身磁共振弥散加权成像和表观弥散系数在恶性淋巴瘤中的应用价值

摘　　要：目的:探讨全身磁共振弥散加权成像(WB-DWI)和表观弥散系数(ADC)在恶性淋巴瘤诊断、疗效监测中的应用价值。方法:回顾性分析经病理明确诊断为恶性淋巴瘤患者40例,其中霍奇经淋巴瘤患者7例,非霍奇金淋巴瘤患者33例;另选健康志愿者11例,行WB-DWI检查。在ADC图上分别测量恶性淋巴瘤患者及健康志愿者淋巴结ADC值,其中恶性淋巴瘤患者于化疗前、第3周期化疗后在初次检查相一致部位再次测量淋巴结ADC值,并进行前后比较,同时与健康志愿者进行对比。结果:化疗前恶性淋巴瘤患者的平均ADC值为(0.67±0.42)×10-3 mm2/s;行第3周期化疗后平均ADC值为(2.36±0.78)×10-3 mm2/s;健康志愿者平均ADC值是(1.91±0.51)×10-3 mm2/s。化疗前和第3周期化疗后ADC值比较,差异具有统计学意义(P<0.05);化疗前和健康志愿者ADC值比较,差异具有统计学意义(P<0.05);第3周期化疗后和健康志愿者ADC值比较,差异无统计学意义(P>0.05)。结论:WB-DWI和ADC值在恶性淋巴瘤的诊断、临床分期及疗效监测中具有一定的指导意义。Objective. To explore the application value of Whole-body diffusion weighted imaging and apparent diffusion coefficient in diagnosis and response assessment of malignant lymphoma. Methods. Retrospective analysis of 40 patients confirmed as malignant tients had Hodgkin,s lymphoma, 33 patients had non-Hodgk teers were also selected. All people underwent WB-DWI scan. lymphoma were conducted, of which 7 pa n, s lymphoma, besides, 11 healthy volun The measurements of ADC values were executed on ADC maps for lymphoma patients and healthy volunteers,lymph nddes, respectively. ADC val- ues were measured before undergoing chemotherapy, and after the third cycle of chemotherapy, respec- tively. The ADC values were calculated and compared. Results. The initial mean ADC value of malignant lymphoma before undergoing chemotherapy was （0.67±0.42） X 10 3rhm2/s, and after the third cycle of chemotherapy the mean ADC value was （2.36±0.78）X 10.3 mm2/s. The mean ADC value of healthy vol- unteers was （1.91±.51）X 10 3 mm2/s. A significant difference in ADC values was found between pa tients with malignant lymphoma before treatment and after the third cycle of chemotherapy（P〈0.05） ; A I significant difference in ADC values was found between patient＇s with malignant lymphoma and healthy volunteers（P〈0.05）. However, there was no significant difference in ADC value between patients with malignant lymphoma that finished the third cycle of chemotherapy and healthy volunteers. Conclusion.. WB- DWI and ADC values have a certain significance in diagnosis, clinical staging and response assessment of malignant lymphoma.
