基牙及义齿支持形式对套筒冠义齿修复的临床效果分析

摘　　要：目的探究并分析基牙类型及义齿支持形式对套筒冠义齿修复效果的影响。方法选择2010年11月至2012年11月在中国中医科学院西苑医院采取套筒冠义齿修复的患者60例,调查其基牙状况、类型及义齿支持形式,观察不同基牙类型及义齿支持形式对套筒冠义齿修复效果的影响。结果修复1个月、6个月、1年后,牙龈指数较修复前无显著变化,差异无统计学意义(P>0.05),探诊深度、碱性磷酸酶活性、牙周袋深度显著低于修复前,且修复1年后的变化最显著(P<0.05);残根残冠组修复后基牙龋损、牙折的发生率分别为8.33%、16.66%,显著高于牙周炎和少数余留牙患者(P<0.05);双侧末端游离者牙折及基托折裂的发生率分别为31.11%、12.05%,显著高于非末端游离及单侧末端游离者(P<0.05)。结论套筒冠义齿对牙列缺损的修复效果较好,且不同牙基类型、不同义齿支持形式修复后的效果有所不同。Objective To explore and analyze the effect of denture abutment types and forms of support for telescopic denture repair. Methods From November 20i0 to November 2012 in China Xiyuan Hospital of Traditional Chinese Medicine, 60 patients who received telescopic denture repair were included in the study, the abutment position, type, and denture support form were investigated, the effects of different types and abutment denture forms on the telescopic denture repair. Results Gingival index was not significantly changed a month, six months, and one year after repair, comparing with before restoration （P 〉 O. 05 ）, probing depth, alkaline phosphatase activity and periodontal pocket depth were significantly lower than before restora- tion and thechanges were most obvious 1 year after the repair（ P 〈 0.05 ） ; residual crown and root group abut- ments caries, tooth fractures incidence rates after repair were 8.33 % , 16.66%, significantly higher than the periodontitis and few remaining teeth patients （ P 〈 0.05 ） ; bilateral distal patients odontoclasis and denture base fracture incidence rates were 31.11%, 12.05%, much higher than the non-free and unilateral distal end ,with statistically significant difference（ P 〈 0.05 ）. Conclusion Telescopic denture on dentition defect repair has good effect, and different types of tooth base and different forms of support have different effects on the repair.
