Tourette综合征儿童T淋巴细胞亚群与情绪的相关性研究

摘　　要：目的检测Tourette综合征(TS)儿童的T淋巴细胞亚群水平,评估TS儿童的情绪特征,探讨TS儿童情绪与细胞免疫指标的关系。方法检测57例TS儿童和43例正常儿童的T淋巴细胞亚群水平,采用儿童焦虑性情绪障碍筛查表(SCARED)及儿童抑郁障碍自评量表(DSRSC)进行情绪评定,并对相关免疫指标与各项情绪评分进行多重线性回归分析。结果 TS组CD4+水平、CD4+/CD8+比值低于对照组(P<0.05)。TS组在躯体化/惊恐、广泛性焦虑、分离性焦虑、社交恐怖、学校恐怖及焦虑和抑郁总分均明显高于对照组(均P<0.05)。多因素相关分析显示TS儿童的CD4+/CD8+比值与躯体化/惊恐、焦虑总分负相关。结论 TS儿童体内存在T细胞免疫水平改变和明显的焦虑、抑郁情绪问题,躯体化/惊恐、焦虑可能是TS儿童体内CD4+/CD8+比值变化的主要情绪影响因素。[ Objective] To detect the level of T-lymphocyte subsets in Tourette syndrome （TS） children, and eval- uate the characteristic of emotion in TS children, and discuss the relationship between emotion and cellular immuni- ty in TS children. [ Methods ] 57 TS children and 43 normal children were collected to determine the level of T- lymphocyte subsets. The two groups were evaluated by the Screen for Child Anxiety Related Emotional Disorders （SCARED） and depression self-rating scale for children （DSRSC）. Multiple linear regression analysis was used to i- dentify the correlation between T-lymphocyte subsets and emotion. [ Results ] The levels of CD4＋ and CD4＋/CD8＋ in TS group were significantly lower than control group （27.11＋5.52 vs 30.22±6.68; 0.91±0.24 vs 1.03±0.25, P 〈0.05）. ＇IS group showed significantly higher scores than control group in somatization/scared, generalized anxiety, separative anxiety, social phobia, school phobia, total scores of SCARED and DSRSC （5.46±3.96 vs 2.42±2.68; 5.61±3.39 vs 2.81±2.78; 5.09±2.87 vs 2.91±1.93; 4.40±3.53 vs 2.86±2.07; 1.51±1.38 vs 0.81±0.98; 22.07±11.38 vs 11.81±8.69; 10.63±4.83 vs6.05±4.56, P 〈0.05）. Multiple linear regression analysis shows the level of CD4＋/CD8＋ in TS children was negatively correlated with somatization/scared and total score of SCARED. [ Conclusions ] There was T-cell im- mune level change in riPS children who also had obvious problems of anxiety and depression. Somatization/scared and anxiety were probably the major related factors to the level change of CD4＋/CD8＋ in TS children.
