摘 要:目的:比较非埋植型和埋植型不同种植方式种植体义齿修复受载后其骨界面中骨形成蛋白2(BMP-2)表达变化的异同。方法:取纯系Beagle犬8只,于每只犬的两侧下颌分别植入非埋植型和埋植型种植体各3枚,每侧第1枚不作义齿修复用于对照,其余各枚均采用钴-铬合金铸造金属全冠修复。分别于修复后2、4、8、12周各随机处死2只实验犬,取下颌相应骨段,在每个种植体周围10mm处制作颊舌向纵行石蜡切片(片厚4μm),采用免疫组化法检测各时期种植体周围骨界面组织中BMP-2的表达水平。结果:两种类型种植体未进行冠修复时,其骨界面组织中BMP-2的表达均呈弱阳性,且不随观察时间延长而变化(P>0.05);而冠修复受载后,两种种植体骨界面组织中BMP-2表达的阳性强度均随观察时间的延长而增加,8周时达到高峰,12周时下降至接近对照组水平,各时间点两两相比差异均有统计学意义(P<0.05);非埋植型与埋植型种植体之间相比,无论是对照组还是实验组,各时间点内两者的BMP-2表达强度均无显著性差异(P>0.05)。结论:非埋植型和埋植型种植体在义齿修复受载后,所承受的机械载荷均能刺激种植体—骨界面组织中BMP-2表达的规律性变化,两种种植体相比无明显差异。AIM: To observe the expression of bone morphogenic protein-2 (BMP-2) in implant-bone inter- face after loading of nonsubmerged or submerged implant dentures. METHODS :8 adult inbred beagle dogs were used to build the animal models. 3 nonsubmerged and 3 submerged implants were implanted into each side of the mandible re- spectively. The first implant without denture at each side was used as the control . The remaining implants were fixed by metal full crown. 2, 4, 8 and 12 weeks after crown restoration, 2 dogs were sacrificed respectively. The corresponding mandible bone segments were taken. Buccolingual longitudinal paraffin sections (with the thickness of 4 μm) about 10 mm to each implant were made. The expression of BMP-2 in the implant-bone interface was examined by immunohis- tochemistry. RESULTS : Weak expression of BMP-2 in the implant-bone interface in the 2 control groups ( without loading, P 〉 0.05 ) and no significant change with time ( P 〉 0.05 ) were observed. In the 2 experimental groups ( with load- ing) strong BMP-2 expression in the implant-bone interface was observed and the expression enhanced with loading time. Peak expression was found 8 weeks after loading, afterwards decreased and returned to the level of the control groups 12 -weeks after loading. No obvious difference of BMP-2 expression in the implant-bone interface was observeu Detween the nonsubmerged and submerged implants at the same time points (P 〉 0.05 ). CONCLUSION: The mechanical loading from the nonsubmerged and submerged implant denture can stimulate the expression of BMP-2 in the implant-bone inter- face, there is no distinct difference between the 2 kinds of implants.
