摘 要:目的:研究安氏Ⅱ类错牙合的年青成人患者应用Forsus矫治器引导下颌向前骨矫形作用颞下颌关节(TMJ)盘-突-窝位置关系的改变,探讨Forsus矫治器对TMJ结构产生的影响。方法:23例骨性安氏Ⅱ类下颌后缩患者(男10例,女13例),平均年龄(19.6±2.1)岁,接受Forsus功能矫治,设计平均疗程9.5个月。治疗前后分别对患者TMJ进行MRI片观察和测量。结果:治疗前后左侧髁突软骨增生约(0.53±0.15)mm,右侧髁突软骨增生约(0.49±0.27)mm,说明髁突发生了适应性改建,尽管关节盘的位置比较靠前(P<0.05),但仍然处于允许的生理范围之内。治疗前后关节盘和髁突的位置无明显改变(P>0.05)。结论:Forsus矫治器引导下颌向前主要刺激髁突产生适应性改建,关节窝改建不明显,且矫治结束后髁突在关节窝内的位置不受矫治器的影响,不会对TMJ功能产生不利影响。AIM: To investigate the changes in the disc-condyle-fossa relationship of temporomandibular joint (TMJ) following mandibular advancement. METHODS : 23 patients ( 10 boys and 13 girls, 19.6 ± 2.1 years old) with class II malocclusions were treated with Forsus appliance for an average time of 9.5 months. At the beginning and after treatment, MRI images of TMJ were obtained and evaluated. RESULTS: The hyperplasia of left and right condyle of the patients was 0.53 ± 0.15 mm and 0.49 ±0.27 mm respectively. Although the articular disc was posi- tioned more anteriorly in the peak pubertal stage compared with its pretreatment position ( P 〈 0.05 ), the position of the disc was still within the physiologic range. No significant change was observed for disc-condyle relationship after treatment (P 〉 0.05 ). CONCLUSION : Forsus appliance can generate adaptive alterations of mandibular condyle, while condylar-fossa relationship has no remarkable change. Forsus appliance has no apparent negative influence on TMJ function.
