中重度广泛型侵袭性牙周炎患者上颌窦黏膜形态变化特点及相关性研究

摘　　要：目的:应用锥形束CT(CBCT)分析中重度广泛型侵袭性牙周炎(GAgP)患者上颌窦黏膜形态变化特点,并进行上颌窦黏膜炎症反应的相关性研究。方法:随机选取经CBCT扫描并诊断为中重度GAgP患者40例作为实验组,无牙周炎患者60例作为对照组。检测实验组患者上颌第二前磨牙、第一磨牙和第二磨牙的牙周探诊深度(PD)和临床附着丧失(CAL),分析实验组和对照组患者上颌窦黏膜形态特点。进行统计学分析。结果:实验组患者上颌窦黏膜存在增厚改变的发生率为92.5%,而对照组为18.3%(P<0.05)。在实验组中,上颌窦底黏膜的最大厚度值(DSM)与上颌磨牙牙槽嵴顶到上颌窦底的骨性边缘的最小垂直距离(HBS)呈负相关。其中,DSM与上颌第一磨牙HBS关系最为密切,右侧和左侧相关系数分别为-0.51和-0.73(P<0.05)。实验组患者DSM与PD和CAL呈弱相关。结论:中重度GAgP患者上颌窦底黏膜容易发生增厚样改变,上颌窦底黏膜增厚反应程度与牙周炎牙槽骨吸收的严重程度关系密切。Objective： To analyse the morphological changes of the schneiderian memberane of maxillary sinus in the advanced generalized aggressive periodontitis （GAgP）subjects using CBCT and analyse the correlation of the inflammation between the degree of GAgP and the schneiderian memberane. Methods： GAgP subjects were selected and scanned by CBCT. 60 normal subjects were set as the control. The probing depth （PD） and the clinical attach ment level（CAL）of the maxillary second premolar, maxillary first molar and maxillary second molar of the test sub- jects were recorded. The morphological changes of the schneiderian memberane were analysed by the software of CBCT. Results： The incidence rate of the mucosa thickening in the test group was 92.5 %, and the compare group was 18.3% （P〈0.05）. The significance negative correlation was found between the maximum dimension of the schneiderian membrance （DSM） and the minimum height of alveolar bone margin to sinus border （HBS） in the test group. There was close relation between the DSM and the HBS of the maxillary first molar. The coefficient of correlation was -0.51 in the right and -0.73 in the left （P〈0.05）. The weak correlation was found between the DSM and the PD and the CAL. Conclusion： The changes of mucosal thickening were easily found in the advanced GAgP subjects, There Was close relationship between the DSM and the severity degree of alveolar bone loss of the periodontitis.
