健脾合剂对脾虚型溃疡性结肠炎患者的免疫干预机制研究

摘　　要：目的:研究健脾合剂如何对脾虚型溃疡型结肠炎(UC)进行免疫干预。方法:选取健脾合剂治疗组100例及健康对照组100例。健脾合剂(党参、白术、山楂、陈皮、麦芽等)由本院中药制剂室协助配制,一剂煎3袋,各150 mL。每天分3次温服,日1剂。均为20天1个疗程,服药2个疗程复查免疫指标。结果:UC患者治疗前的CD+4细胞与对照组相比无显著性差异,而CD+3和CD+8较对照组显著减少:经治疗临床症状缓解后,其CD+4细胞数回落(与治疗前比较,P>0.05),CD+3和CD+8细胞数回升(与治疗前比较,P<0.05和P<0.05)。UC患者治疗前的CD+4/CD+8比值较对照组显著升高(P<0.05),经治疗后回落(与治疗前比较,P<0.05)。两个疗程间总有效率未见显著性差异。结论:健脾合剂对溃疡性结肠炎有显著的免疫调节作用,且主要提高了Ts细胞,从而缓解自身免疫及变态反应所致的系列临床症状。Objective：To study the immune intervention efficacy of the spleen -strengthening mixture on spleen deficiency type ulcerative colitis. Methods： Spleen -strengthening mixture treatment group had 100 cases and 100 cases healthy as the control group. The spleen - strengthening mixture （ Dangshen mixture, atractylodes, hawthorn, dried tangerine or orange peel, mah, etc ）. was used,three bags as a dose,150 mL for each,three times a day and one dose a day. 20 days was a course of treatment,and after 2 courses, the immune indexes were reviewed. Results ： The CD4^＋ cells had no significant difference in treatment group compared with control group, while CD3^＋ and CD8^＋ significantly reduced than that of control group. After relief of clinical symptoms, the CD4^＋ cell count dropped （compared before treatment, P 〉 0. 05 ） , the number of CD3^＋ and CD8^＋ cells recovered, compared with before treatment,P 〈 0. 05 and P 〈 0. 05 ）. CD4^＋/CD8^＋ ratio of UC patients before treatment significantly increased than that of the control group （P 〈 0. 05 ）, and reduced （compared with before treatment） after treatment（ P 〈 0. 05 ）. Total effective rate between the two treatments had no obvious difference. Conclusion ： The spleen - strengthening mixture on the immunoregulatory effects of ulcerative colitis is significant, and mainly improves Ts cells, so as to relieve the UC of autoimmunity and allergy caused by a series of clinical symptoms.
