鼻咽癌放疗后鼻部并发症的个案分析

摘　　要：目的探讨鼻咽癌(nasopharyngeal Carcinoma,NPC)放疗后鼻部并发症的临床特点及诊治方法。方法对NPC放疗后鼻出血(nosebleed,NB)患者1例,NB并鼻窦炎(sinusitis)1例进行诊治,并回顾性分析该2例患者的病因及治疗方法。结果患者一号在多次填塞止血无效后采用鼻内镜(Nasal endoscopy,NE)下电凝止血,疗效显著,且随访1年未复发。患者二号采取NE下激光止血,并使用抗生素联合鼻腔冲洗治疗鼻窦炎,疗效显著,随访至今未复发。结论 NE下应用激光以及电凝法止血能有效治疗NPC放疗后并发NB,抗生素联合鼻腔冲洗治疗鼻窦炎,疗效显著,值得推广。Objective To investigate the clinical characteristics and diagnosis and treatment methods of complication of NPC after radiotherapy. Methods Nasal hemorrhage with NPC after radiotherapy NB in 1 patients, NB and sinusitis in 1 cases, to diagnosis and treatment. And retrospectively analyzed the etiology and treatment of 2 patients. Results The patients in multiple packing invalid using endoscopic NE by electric coagulation hemostasis, the curative effect was distinct, and were followed up for one year without recurrence. Patients took NE two laser hemostasis, had remarkable curative effect and the use of antibiotics in treatment of sinusitis, nasal irrigation, follow-up no recurrence. Conclusion Laser and electric coagulation hemostasis with energy efficiency in the treatment of NPC after radiotherapy NB application under NE, the treatment of sinusitis, nasal cavity flushing of antibiotics combined effect is significant, worthy of promotion.
