电视胸腔镜辅助下小切口手术治疗非小细胞肺癌的临床观察

摘　　要：目的探讨电视胸腔镜辅助下小切口手术治疗非小细胞肺癌的效果。方法将106例肺癌患者分为对照组(60例)和观察组(46例),对照组患者接受传统开胸手术,观察组患者接受VAMT术,对比分析2组患者的疗效。结果①2组患者手术时间相比差异无统计学意义(P>0.05);观察组开胸时间、术后下床活动时间、住院时间、术中出血量、术后第1天引流量、术后疼痛VAS评分低于对照组(P<0.05)。②观察组手术并发症发生率低于对照组(P<0.05)。③随访18个月,2组患者复发率、转移率差异无统计学意义(P>0.05)。2组患者随访期内均无死亡病例。结论VAMT术治疗非小细胞肺癌具有创伤小、恢复快、并发症少、术后生活质量高等优点,值得临床推广应用。Objective To investigate the efficacy of video-assisted mini-thoracotomy （VAMT）for non-small-cell lung cancer. Methods 106 cases of non-small-cell lung cancer patients were divided into the control group （n = 60） and the observation group （n = 46）. The control group received the traditional open chest operation, and the observation group received VAMT, curative effects of the 2 groups were compared. Results ①The operation time of the 2 groups had no statistical difference （ P 〉 0.05 ） , and the open chest time, postoperative ambulation time, hospitalization time, amount of intra-operative bleeding, drainage volume of postoperative 1 st day and postoperative pain scores of VAS of the observation group were significantly lower than those of the control group（ P 〈 0.05 ）. ②The incidence rate of operation complications of the observation group were significantly lower than that of the control group. ③All patients were followed up for 18 months, and no death case was found during the follow-up period, and there were no statistical difference in recurrence rate and metastasis rate between the 2 groups （ P 〉 0.05 ）. Conclu- sion The video-assisted mini-thoracotomy for non-small-cell lung cancer is of less trauma, rapid recovery,less complications, and higher quality of life, it is worthy of clinical application.
