摘 要:目的应用锥体束CT(CBCT)观察磁力扩大上颌牙弓后后牙牙根的吸收情况。方法选取采用磁力扩弓矫治器进行上颌扩弓的32例患者,拍摄扩弓前后CBCT,测量上颌后牙牙根长度,采用配对t检验分析扩弓前后牙根长度的变化,采用成组t检验分析不同发育阶段的牙根长度的差异。结果磁力扩弓后,支抗牙较非支抗牙更易出现牙根吸收。根尖孔已闭合组(G2组)上颌后牙牙根长度有减少的趋势,第一磨牙近中颊根长度减少,第一前磨牙颊根减少。根尖孔未闭合组(G1组)后牙牙根未见吸收。结论磁力扩弓对支抗牙的牙根长度有影响。G1组的支抗牙对于矫形力的耐受强于G2组。早期进行扩弓治疗可能降低牙根吸收的风险。Objective To investigate the root length of posterior teeth after palatal expansion by magnetic expansion appliance. Methods A total of 32 patients who accepted palatal expansion with magnetic expansion appliance were enrolled in the study. Pretreatment and post-treatment cone-beam computed tomography (CBCT) images were obtained. A paired t test was used to evaluate the length change, and a group t test was used to compare the tooth lengths of developing and developed teeth. Results Anchorage teeth showed greater root resorption than non-anchorage teeth, whereas the developed posterior teeth showed reduced resorption. Moreover, the mesial buccal roots of the first molar and first premolar were reduced. No resorption was found in the developing teeth group. Conclusion The magnetic palatal expansion can reduce the root length, particularly of developed root. The developing root of anchorage teeth show greater tolerance to resorption than the developed root. Early palatal expansion treatment with magnetic expansion appliance can reduce the risk of root resorption.
