摘　　要：目的 探讨腮腺少见疾病的影像特点,提高对其的认识水平.方法 回顾性分析经病理证实的20例腮腺少见疾病的临床及影像资料.结果 （1）炎性病变与良性肿瘤在肿块边缘（P=0.041）、浅层筋膜（P=0.010）、皮下脂肪（P=0.001）、局部皮肤（P=0.001）及淋巴结（P=0.007）的表现差异有统计学意义;炎性病变与恶性肿瘤在局部皮肤（P=0.010）及皮下脂肪（P=0.010）的表现差异有统计学意义;良性肿瘤与恶性肿瘤在肿块边缘（P=0.032）及浅层筋膜（P=0.032）的表现差异有统计学意义.（2）神经鞘瘤为边缘清晰、包膜完整的实性或囊实性肿块;木村病为边界清或不清的块影,伴多发淋巴结;结核表现为肿块及淋巴结均呈环形强化;良性淋巴上皮病变为腮腺弥漫性肿大伴絮状、结节状影;肌上皮瘤为边缘清晰的分叶状肿块.结论 仔细分析腮腺少见病的影像表现,能提示病变的性质;部分腮腺少见病具有一定的影像特征,结合临床,能准确诊断.Objective To investigate the imaging features of uncommon diseases in the parotid gland and to further characterize recommendations for its diagnosis. Methods The imaging and clinical data of 20 patients with pathology confirmed uncommon parotid gland disease were retrospectively analyzed. Results ①Specific features which could be used to distinguish inflammatory lesions from benign tumors were as following： margin （P=0. 041）, shallow fascia （P=0. 010）,, subcutaneous tissue （P=0. 001）, local skin （P=0. 001） and lymphadenopathy （P= 0. 007）. There were obvious differences between inflammatory disease and malignant tumor in local skin （P=0. 010） and subcutaneous fat （P=0. 010）. The manifestation of margin （P=0. 032） and shallow fascia （P =0. 032） had significant difference between benign and malignant tumor. ②Schwannoma was a kind of solid or cystic solid mass which had well-defined margin and intact cover. Kimura＇s disease was a lesion with clear or unclear margin, frequently associated with regional lymphadenopathy. Tuberculosis presented ring-shaped enhanced mass and lymph node. Benign lymphoepithelial lesions showed diffuse enlargement of the parotid gland with nodules. Myoepithelioma was a kind of lobulated mass with well-defined margin. Conclusion Parts of the uncommon parotid gland diseases have certain imaging characteristics, which can be accurately diagnosed by combination with clinical data.
