摘　　要：目的 探讨改良单纯排粪造影对出口梗阻型便秘的鉴别价值.方法 对271例有不同程度排便困难的患者进行改良单纯排粪造影检查.观察动静态排便过程,摄取患者静息、提肛、力排充盈相和黏膜相,加摄充气后腹部立位片,在PACS系统上精确测量,并根据诊断标准得出完整诊断结果.结果 271例患者中,顺利完成检查者267例（98.5%）,其中121例（45.3%）患者改良单纯排粪造影表现正常,146例（54.7%）异常,常为多种疾病并存.改良后发现横结肠冗长11例（4.1%）,脾曲综合征12例（4.5%）,结肠张力偏低11例（4.1%）,升结肠癌1例,结肠息肉2例,结肠憩室1例,不完全性结肠反位1例.结论 排粪造影仍是诊断出口梗阻型便秘的主要检查方法,而检查技术的改良,有助于不同类型的鉴别及隐匿复杂型的检出.Objective To explore the differential diagnostic value of improved simple defecography for outlet obstruction constipation. Methods 27 l patients who had difficult of defecation in different degrees were underwent defecography examination. Their dynamic and static defecate process were observed. Showed the full-filling and defecation image when patient was resting, lifting anus, defecating. Then, take abdomen vertical solice after aerating. According to the diagnostic criteria, the integral results were received. Results In 271 cases, 267（98.5%） were successfully complete the defeeography examination. 121 （45.3%） showed normal in this exam,146（54.7%） were anomalous and always with a variety of intestinal dysfunction. After improving, we found 11 （4.1 % ） cases with colon t ransversum lengthy, 12 （ 4.5 % ） cases with splenic flexure syndrome, 11 （ 4.1 % ） cases with colon tension lower, 1 case with colon ascendens cancer, 2 cases with colon polypus, 1 case with colon diverticulum, l case with incomplete reversed colon. Conclusion Defecography is the primary method for diagnosing outlet obstruction constipation, and the improved inspection technique is helpfuI fur finding the different and complex types.
