摘 要:目的探讨Transglutaminase-2(TG2)蛋白在肝细胞肝癌(肝癌)组织中的表达情况及其临床意义。方法本回顾性研究标本为2006年5月至2010年7月在中山大学附属第三医院行肝癌肝切除的51例患者的肝癌标本(肝癌组)。所有患者均签署知情同意书,符合医学伦理学规定。其中男44例,女7例;年龄(46±10)岁。另术中取21例癌旁组织作为对照(对照组)。采用免疫组织化学方法检测两组标本中TG2蛋白的表达情况。两组标本中TG2蛋白表达比较及TG2蛋白表达与患者临床病理学参数的关系分析采用χ2检验或Fisher确切概率法,TG2蛋白表达与患者临床病理学参数相关性分析采用Spearman秩相关。结果肝癌组TG2阳性率为53%(27/51);对照组阳性率为14%(3/21),差异有统计学意义(χ2=9.145,P<0.05)。肝癌组织TG2蛋白表达与AFP、肿瘤直径、血管浸润及肿瘤、淋巴、转移(TNM)分期呈正相关(r=0.357,0.497,0.750,0.747;P<0.05),而与肿瘤组织学分化程度呈负相关(r=-0.438,P<0.05)。结论 TG2蛋白在肝癌组织呈高表达,其表达与肿瘤直径、血管浸润及TNM分期呈正相关,提示TG2可能在肝癌的发生、发展和转移中发挥着重要作用。Objective To investigate the expression and clinical significance of Transglutaminase-2 (TG2) protein in hepatoeellular carcinoma tissues. Methods Samples of hepatocellular carcinoma (HCC) tissues from 51 patients (HCC group) who underwent hepatectomy from May 2006 to July 2010 in the Third Affiliated Hospital of Sun Yat-sen University were collected in this retrospective study. The informed consents of all participating subjects were obtained and the local ethical committee approval had been received. There were 44 males and 7 females with a mean age of (46+10) years old. Samples of pericarcinomatous tissues from 21 cases were collected during operation as control (control group). The TG2 protein expression in two groups was detected by immunohistochemistry. The comparison of TG2 protein expression in two groups and the relation between TG2 protein expression and clinical pathology parameters of patients were analyzed by Chi-square test or Fisher exact probability. And the correlation between TG2 protein expression and clinical pathology parameters of patients was analyzed by Spearman rank correlation analysis. Results The positive rate of TG2 expression was 53%(27/51) in HCC group and 14%(3/21) in control group, and there was significant difference between two groups(χ2=9.145, P〈0.05). The TG2 protein expression was positively related with AFP level, tumor diameter, vascular invasion and tumor, node, matastasis (TNM) staging (r=0.357, 0.497, 0.750, 0.747; P〈0.05), while was negatively related with tumor histological differentiation (r=-0.438, P〈0.05). Conclusions The TG2 protein expression in HCC tissues is high and is positively related with tumor diameter, vascular invasion and TNM staging, which indicates it may play an important role in the development, progression and metastasis of HCC.
