Toric人工晶状体植入术矫正高度角膜散光的视觉质量

摘　　要：目的观察AcrySof Toric散光人工晶状体植入术矫正高度角膜散光术后的视觉质量,评估这项技术的安全性、有效性和稳定性。方法选择于我院行超声乳化白内障吸除联合人工晶状体植入的患者20例,术眼角膜规则散光为3.0~4.5 D。观察术后1 d^6个月的裸眼视力、最佳矫正视力、残余散光,对比敏感度、像差、散射光、人工晶状体旋转度等,评价Toric人工晶体植入矫正高度角膜散光的安全性、有效性和稳定性。结果术后6个月,患者裸眼视力0.07±0.16(LogMAR),总残余散光(0.42±0.34)D,明显小于术前(P<0.05),全眼的散射光为1.18±0.19,与术前比较有统计学差异(P<0.05)。术后远用镜脱镜率为95%,90%的患者晶状体轴位旋转<4°。结论散光人工晶状体植入可矫正高度角膜散光,提高患者术后的裸眼视力,减少对眼镜的依赖,并具有良好的旋转稳定性,可长期保持良好的矫正作用。Objective To observe the postoperative visual quality of high eomeal astigmatism corrected by AcrySof Tofic lens implantation, and evaluate the safety, effectiveness and stabillity of this technique. Methods A total of 20 patients with preoperative reegular astigmatism of 3.0-4.5 di- opters （ D ） were recruited in this clinical study. Phacoemulsification and AcrySof Tofic lens implantation was performed on these patients. Postopera- tive uncorrected visual acnity （ UCVA ） from one day to six months, best corrected visual acnity, residual refractive astigmatism, contrast sensitivity, aberration, straylight, and the lens misalignment were measured and analyzed. Results Six months postoperatively, the mean UCVA was 0.07±0.16 （ IgMAR ） , which was significantly different from preoperative UCVA. Meanwhile, refractive cylinder decreased significantly with a mean residual refractive astigmatism of 0.42±0.34 D （ P 〈 0.05 ）. The eye straylight was 1.18_0.19 （ P 〈 0.05 ）. The postoperative distance spectacle-independence rate was 95.0%. The IOL misaligrnment was less than 4 degrees in 90% patients. Conclusion AcrySof Tofic intraoclular lens was effective in cor- recting high corneal astigmatism, which improved the postoperative UCV A, and reduced the spectacle dependence. This technique provided not only excellent stahihty but also good long term eorrection effeect.
