消痰散结方联合化疗治疗晚期胃癌患者32例临床观察

摘　　要：目的观察消痰散结方联合化疗治疗晚期胃癌患者的临床疗效及可能作用机制。方法将67例晚期胃癌患者随机分为治疗组32例和对照组35例。对照组给予口服替吉奥胶囊联合静脉滴注奥沙利铂方案化疗,治疗组在对照组的基础上加用消痰散结方,每日1剂。两组均连续治疗21天为1个疗程,共4个疗程。观察两组患者治疗前后外周血中N糖的变化,治疗后评价近期疗效。结果治疗组近期疗效总有效率为59.38%,肿瘤控制率为84.38%;对照组总有效率为45.71%,肿瘤控制率为80.00%,两组比较差异均无统计学意义(P>0.05)。与治疗前比较,治疗后治疗组N糖峰值Peak3、Peak6和Peak7显著升高(P<0.05或P<0.01),Peak1、Peak2、Peak5和Peak9显著降低(P<0.01);对照组Peak3、Peak6和Peak7显著升高(P<0.01),Peak5和Peak9显著降低(P<0.01)。治疗后治疗组Peak1、Peak2明显低于对照组(P<0.05或P<0.01),Peak4明显高于对照组(P<0.05)。结论消痰散结方联合化疗治疗晚期胃癌临床有效,可调节外周血中N糖的表达。Objective To observe the therapeutic effect and mechanism of Xiaotan Sanjie Formula （formula to disperse phlegm and dissipate binds） combined with chemotherapy for advanced gastric cancer. Methods Totally 67 eases of advanced gastric cancer were randomized into the treatment group of 32 cases and control group of 35 cases. Both groups were given DeFazio capsules orally and intravenous infusion of oxaliplatin. The treatment group was added with Xiaotan Sanjie Formula one dosage daily. A 21-day continuous treatment was one course and the treatment lasted for four courses in both groups. The serum N-glycan was observed before and after treatment. The therapeutic effect was evaluated after treatment. Results The total effective rate and tumor control rate was 59.38% and 84.38% in the treatment group and 45.71% and 80.00% in the control group respectively, with no significant difference between groups （P〉0.05）. The Peak3, Peak6 and Peak7 values of serum N-glycan were significantly increased but the Peak5 and Peak9 values of serum N-glycan were significantly decreased in both groups （P〈0. 01 or P〈0.05）. The Peakl and Peak2 values of serum N-glycan were significantly decreased in the treat- ment group （P〈0.01）, with a significant difference between groups （P〈0.01 or P〈0.05）. The Peak4 value of serum N-glycan in the treatment group was significantly higher than that in the control group （P〈0. 05）. Conclusion Xiaotan Sanjie Formula combined with chemotherapy is effective for advanced gastric cancer and it can regulate the expression of serum N-glycan,
