Effects of Mesalazine Combined with Xileisan and Yunnanbaiyao Powder and Dexamethasone for Retention Enema in the Treatment of Ulcerative Colitis

摘　　要：目的探讨美沙拉嗪联合锡类散、云南白药及地塞米松保留灌肠治疗溃疡性结肠炎(UC)的疗效。方法144例UC患者随机分为对照组和治疗组,每组各72例,对照组给予美沙拉嗪1.0 g/次,3次/d,治疗组在对照组基础上联合锡类散1.0 g,云南白药2.0 g,地塞米松5 mg保留灌肠,每晚1次,疗程为4周,比较两组患者的临床疗效及结肠镜下表现,采用Mayo疾病活动指数进行评分,并比较治疗期间两组患者不良反应率。结果治疗组临床疗效明显优于对照组(P<0.05);两组Mayo疾病活动指数均下降,但治疗组下降更明显(P<0.05);两组治疗前后结肠镜下评分比较,治疗组在促进黏膜修复、愈合方面明显优于对照组(P<0.05)。结论应用美沙拉嗪口服联合锡类散、云南白药及地塞米松保留灌肠治疗溃疡性结肠炎疗效确切,值得临床推广。Objective To investigate the effects of mesalazine combined with Xileisan and Yun- nanbaiyao powder and dexamethasone for retention enema in the treatment of ulcerative coli- tis.Methods 144 patients with ulcerative colitis were randomly divided into two groups, 72 patients in the treatment group were given mesalazine 1.0 g three times daily before meals,Xileisan 1.0 g,Yunnanbaiyao powder 2.0,and dexamethasone 5mg for retention enema for 4 weeks. 72 patients in the control group were given mesalazine 1.0g three times daily before meals for 4 weeks. The improvement of patients＇ clinical symptom and manifestation under colonoscope were observed after hospitalization and four weeks after medication.Results Clinical efficacy index score, the difference of reduction was statistically significant （t= 2.215,P〈0.05）. Mayo disease activity index score, the difference of reduction was statistically significant（t= 2.102,P〈 0. 05）.Index score under colonoscope, the difference of reduction was statistically significant （t =2.084,P〈0.05）. The adverse reaction rate was monitored for 4 weeks after treatment, which was 27.78% and 11.12% for treatment group and control group respectively.There was no significant difference between the two group （χ2 = 0. 936, P〉0.05）. Conclusion Mesalazine combined with Xileisan and yunnanbaiyao powder and dexamethasone for retention enema had good clinical effect in the treatment of ulcerative colitis.
