摘　　要：目的探讨第四脑室肿瘤的显微手术技巧及并发症预防。方法回顾性分析21例显微手术治疗的第四脑室肿瘤患者的临床资料。结果肿瘤全切除16例,次全切除3例,大部分切除2例。术后头痛、头晕症状均明显改善;脑积水均缓解;缄默症2例;无手术死亡病例。结论术中注意保护第四脑室底部,防止损伤小脑后下动脉及其分支,熟练的显微外科技术和积极预防手术并发症,是取得良好疗效的关键。Objective To investigate the microsurgical technique and prevention of postoperative complications of the fourth ventricle tumors.Methods The clinical data of 21 patients with the fourth ventricle tumors from July 2009 to June 2012 were retroperatively analyzed.Results Of these 21 patients treated with microsurgery,tumors in 16 were totally removed,subtotally in 3,and partially in 2.The symptoms of headache and hydrocephalus were all relieved,compared with preoperatively.Postoperative complications included 2 of cerebellar mutism syndrome.There was no death due to the operation.Conclusion Protection of the floor of the fourth ventricle and no injury to posterior inferior cerebellar artery,skillful microsurgical technique,prevention of postoperative complications are key points in improving the curative effects of the fourth ventricle tumors.
