腰池置管持续引流治疗脑脊液漏及颅内感染疗效观察

摘　　要：目的观察分析使用腰大池置管持续引流治疗脑脊液漏与颅内感染患者的临床效果。方法从我院2004-09—2012-09收治入院的脑脊液漏与颅内感染患者中抽取40例,随机分为观察组与对照组,在常规综合治疗基础上观察组实施腰大池置管持续引流与鞘内注药,对照组实施反复腰穿与鞘内注药,对比观察2组临床疗效。结果观察组治疗总有效率明显高于对照组,临床症状缓解及感染控制时间均明显低于对照组,治疗5d后颅内压明显低于对照组,差异有统计学意义(P<0.05)。结论对脑脊液漏与颅内感染患者实施腰大池置管持续引流治疗,能够快速有效缓解患者临床症状、控制感染、降低颅内压,明显提高临床疗效,减少重残及致死率,具有进一步推广应用价值。Objective To observe and analyze the clinical effect of using continuous lumbar cisterna drainage treating pa- tients with cerebrospinal fluid leakage and intracranial infection. Methods 40 cases of patients with cerebrospinal fluid leak and intracranial infection from 2012 September to 2004 September in our hospital were selected, and they were randomly divided in- to observation group and control group, the observation group were treated with the continuous lumbar cisterna drainage andintrathecal injection based on routine therapy, control group were treated with repeated lumbar puncture and the sheath injec- tion. The the clinical efficacy of two groups was compared. Results The total effective rate of the patients in the observation group was significantly higher than that of the patients on the control group, and the clinical symptoms and infection control time were significantly lower than those in the control group, intracranial pressure of observation group was significantly lower than that of the control group after 5d treatment,the difference was significant（P〈0.05）. Conclusion Treating patients with the CSF leak and intracranial infection with the implementation of continuous lumbar cisterna drainage treatment can more ef- fectively relieve the clinical symptoms , control infection , reduce intracranial pressure, and significantly improve the clinical ef- ficacy, reduce disability and death number,so it has a further promotion and application value.
